What You Will Do

As an Associate with St. Vincent's Health Services-, you will have the opportunity to

Researches, analyzes and reconciles patient account receivables, refunds, claims processing, reimbursement practices and/or revenue cycle management.

Responsibilities:

  • Processes claims including claims generation, data entry, edits and follow-up to ensure proper billing practices.

  • Maintains and applies knowledge of current regulatory and departmental requirements.

  • Evaluates accounts and prepares adjustments for debit/credit and corrects discrepancies as necessary.

  • Assists with orienting, training and mentoring team members. Supports other staff members as needed.

  • Resolves issues related to claim components, such as erroneous or late charges, invalid coding and invalid insurance information.

  • Follows through on aged unpaid accounts in order to facilitate payment. Takes appropriate action to resolve issues preventing or delaying payment.

  • Consults with team leader and/or management to resolve problematic claims and accounts as well as supports other staff members as needed

  • Reviews payment denials and discrepancies identified through EOB, Remittance Advices or Payer correspondence and take appropriate action to correct these accounts.

  • Attends meetings with payer representatives and/or vendors to address outstanding issues and learn about new regulations and guidelines.

  • Contacts the assigned payer representatives to determine when payment will be made and if any other information is required to adjudicate the claim.

  • Supports manager and department with special projects and other related tasks as assigned including supporting documentation to substantiate internal and external audits.

  • Other Duties as assigned

What You Will Need

Education and Work Experience:

  • HS or Equivalent. Associate's Degree Preferred

  • Minimum 1-2 years experience in medical billing and/or collections. Expereince in healthcare environment preferred. Medical Coding billing a plus. Proficiency with Microsoft required.

EEO/AA/Disability/Veteran Summary: The Agency Patient Account Representative (Agency PAR) functions as the patient representative accountable for the coordination of activities and the navigation of the patient through the financial assistance application process. They work independently and under varying circumstances to address the specialized needs of our self pay, underinsured and undocumented resident patient population to identify resources for assistance that will protect the patient and the Hospital from unnecessary financial loss. The Agency PAR is responsible for the effective outcome on accounts, being proficient and up to date in State and Federal regulations in order to successfully navigate various Social Service agencies (i.e. the Department of Social Services, Social Security, Pulmonary Disease Unit, etc.) to expedite prompt resolution from the point of initial contact through final determination. Working in collaboration with the Care Coordination and Social Work Departments, the Agency PAR formulates solutions to respond to non-clinical customer requests, resolving issues and obstacles while meeting the changing demands and priorities of the hospital and third party environments. The Agency PAR is also responsible to identify trends and offer solutions to improve customer satisfaction and ensure a positive patient experience under sometimes difficult and life changing situations. Responsibilities: 1. ADVOCATES AS A PATIENT REPRESENTATIVE - Financially counsels self pay and under insured patients and/or their representatives, utilizing guidelines for all state and federally funded medical assistance programs as well as Hospital based Free Care, Sliding Scale, Restricted Bed Funds and Emergency Medical for our undocumented resident patient population.. 1.1. Establishes relationships with the patient and/or their representative, advising them of potential funding options available and the subsequent impact of obtaining them on their personal financial status. Works closely and in detail with the patient to aid in making appropriate and beneficial financial decisions. 1.2. Assists in navigating the patient/representative through the medical assistance process by making the initial contact with agencies, completing application forms, securing the required documentation for an acceptance determination and being a resource to the patient on any financial concerns/questions. 1.3. Reviews prior account history to identify past trends and set appropriate guidelines for the most efficient and effective course of follow-up for the patient. 1.4. Institutes follow up with the patient/representative, their legal counsel and the assigned Department of Social Services worker to ensure accuracy, compliance and a timely decision determination. Takes necessary steps in an effort to achieve a positive outcome for both the patient and the Hospital. 1.5. Intervenes in the application process when the patient has shown the tendency to not follow through on requirements. Conveys the urgency of such and strongly and sensitively persuades the patient to work in their best interest to ensure completeness of the application process. 1.6. Exhibits independent decision making skills and exercises sound judgment in the performance of duties, often while encountering new and complex circumstances. 1.7. Works on the behalf of the patient with the Legal and Risk Services Department on cases that require court hearings, appeals and conservator appointments. 1.8. Effectively communicates appropriate information to clinical and other Hospital departments, as well as legal, HFI, Health System and management staff. Ensures that the patient plays an integral role in the application process and is kept updated to developments as they occur. 2. NAVIGATES THE PATIENT THROUGH THE MEDICAL ASSITANCE APPLICATION PROCESS AND COORDINATES THE ACTIVITIES OF ACCOUNTS - Establishes and maintains open communication with the patient and/or their representative, as well as with various government and social agencies and other Hospital departments to efficiently and successfully navigate the patient through the assistance process. Works in collaboration with clinicians, Care Coordinators, Social Workers, nursing homes and other healthcare facilities in order to facilitate timely resolution. 2.1. Coordinates the application process from initial submission through determination serving as a point of contact for the patient and/or their representatives on medical assistance and financial matters. 2.2. Consistently enters all information into SDK accurately at the time of interview to ensure timely follow-up on the account and a thorough account history. Scans all documents obtained within two business days as documented in SDK and evident in SRM. 2.3. Refers all completed applications and necessary documentation to the Department of Social Services workers/district offices within two business days as documented in SDK. 2.4. Identifies and effectively communicates barriers to both the medical assistance application and internal and external daily operations. Offers suggestions for change and works in collaboration with management and other Hospital departments toward resolution. 2.5. Reviews determinations for appropriateness and processes grants/denials within 24 hours of receipt, referring to TCORS, HFI and Free Care when applicable. Maintains departmental guidelines to ensure timely referrals to Collections in order to reduce self-pay liability 2.6. Works in collaboration with Social Work, Care Coordination and the Outlier Management Team to daily monitor activity on in-house accounts in the effort of reducing length of stay and unnecessary delays in patient placement. 2.7. Initiates contacts with the patient, their legal representation, clinicians and other Hospital departments to ensure the timely obtaining of required documentation. 2.8. Conducts consistent and timely follow up with the Department of Social Services, legal representation and various social agencies to ensure appropriate steps are being taken on the patient's behalf to result in an equitable decision on the patient's case. 3. SUPPORTS ORGANIZATIONAL AND DEPARTMENTAL GOALS ? Performs all functions in accordance with established policies and procedures. Consistently and actively participates in in-services and training courses offered through the department, Hospital and the IFE in order to ensure knowledge is current and applied appropriately. 3.1. Maintains current knowledge of State, Federal, and Hospital funded assistance program guidelines to ensure proper use and compliance of regulations when making applications for assistance. 3.2. Reviews all memos and State policy updates as received. Demonstrates and applies a sound knowledge of these policies and procedures, identifying any obstacles or changes that may require a change in our internal processes. 3.3. Keeps informed of billing requirements (i.e. the rebill process, spenddowns), payment policies and changes in third party payer regulations. Makes certain that the appropriate steps are taken to ensure the timely and accurate billing of the accounts receivable. 3.4. Identifies self-pay and large dollar accounts, exercising independent judgment in following department guidelines in order to resolve for payment resource. Properly audits all previous and closed accounts for past payment/non payment history and trending. 3.5. Analyzes and responds to internal and external mail and correspondence within two business days of receipt. Takes corrective actions as needed to facilitate efficient handling and payment. 3.6. Attends and actively participates in weekly staff meetings and designated PFAS workgroups to support team building and enhancements within the department. 3.7. As needed, provides assistance and coverage to other Access and Health System areas. 4. PROMOTES EXCELLENCE IN CUSTOMER SERVICE AND THE I AM YALE NEW HAVEN PLEDGE THROUGHOUT THE ORGANIZATION- Works with Hospital and Health System staff to promote service excellence while coordinating the medical assistance application process. Manages changes in process in order to more efficiently service our customers and maintain high productivity standards. 5.1. Maintains a strong working knowledge of SDK, SCM, POS-C/SRM, On Line Eligibility, DSS Verification systems and Groupwise email lists in order to support and efficiently manage the financial assistance process. 5.2. Utilizes Microsoft Office software to collect data and accurately track proficiency and productivity statistics 5.3. Attends appropriate training and refresher courses offered by the department to broaden knowledge, identify available resources and more efficiently utilize existing systems 5.4. Utilizes more advanced technology such as tablets, WOWs and Search America software to conduct real time bed side interviews to more efficiently complete the medical assistance application Other information: EDUCATION: Associate Degree in Business, Health Administration/Health Services, or related field required or relevant strong customer service/technology/healthcare equivalent experience EXPERIENCE: Three (3) or more years experience in an Access/Healthcare related environment with a strong customer service background. Patient interviewing and hospital third party experience preferred. SPECIAL_SKILLS: Self directed, well organized and exhibiting excellent interpersonal and team oriented skills with the ability and desire to educate and advocate for patients and their families under often difficult conditions. To ensure positive outcomes for both the patient and the Hospital, must be able to multitask and identify the necessity to reprioritize and modify daily operations to respond to volume, immediate need and changes in regulations and guidelines. Excellent investigative and critical thinking skills with the aptitude to identify obstacles and implement actions to resolve. Must have excellent oral and written communication skills with the ability to communicate complex requirements across clinical and financial disciplines. Comprehensive PC proficiency, keyboarding skills and the capability to navigate various software and spreadsheet systems ACCOUNTABILITY: Accountable for the coordination and navigation of the patient and/or their representatives through the financial assistance application process. Responsible for the onsite interviewing of patients, prior, during or post discharge and the obtaining of verifications required for determination. Accountable for the collaboration with clinicians, patients, state/federal medical assistance agencies and various Hospital staff to secure a payment sources for balances not covered by third party payers. Must be available to work flexible shifts to meet patient needs including weekends and alternating holidays. COMPLEXITY: Based on often unusual and complex circumstances, requires an in-depth knowledge of state and federal regulations and requirements in order to successfully assess situations and identify appropriate resources for assistance. Must be capable of discretion and independent judgment when dealing with a variety of situations and communicating with patients, families, clinicians and other hospital staff.
