What you can expect:
Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying the service excellence goals of care, concern, respect and cooperation. As a contributing member of a health care team, develops and provides a comprehensive plan of care for assigned patients.
How you will make an impact:
Coordinates the development of a health care plan for the Womens Health patient which includes performing a risk assessment and assigning risk to all OB intakes and establishes priorities for the patient as a member of the interdisciplinary team
Works directly with the care teams on program specific patient care, individualized patient care plans and fulfilling of provider directed tasks.
Implements and documents in E.H.R. all patient care activities based on program protocols or policies.
Tracks all patient contact from initial perinatal visits through post-partum to assure continuity of care.
Collaborates directly with Womens Health providers in ongoing implementation of health care needs of the patient.
Provides initial prenatal education that includes prenatal care course, counseling on tests, expectations, patient responsibilities, and management.
Initiates Womens Health referrals for diagnosed patients.
Implements standing orders for initial labs, tests, according to established protocols and provides follow up on abnormal labs.
Communicates with affiliated hospital or referral institutions per provider request for: OB records on a monthly basis; Faxes appropriate documents that are necessary for continuity of care.
Assists in collection, computing and interpreting data for grant reporting.
Maintains personal contact with patients as needed to ensure compliance with prescribed treatment plans.
Tracks and manages Womens Health compliance for appointments, referrals, and consultations.
Addresses all normal and abnormal labs and provides education. Reports al STIs as appropriate based on CDC guidelines and STD screening guidelines.
Follows up outcome of high-risk patients, transfers to tertiary centers (e.g., University of Illinois, University of Chicago, and Cook County Hospital).
Provides nursing level patient education for all Womens Health issues
Will participate in the disease management or performance improvement process through either task force participation, including Womens Health task force or implementation of approved protocols and disease management practice.
Adhere to Howard Brown Human Resources Policies and Procedures.
Actively participates in fulfillment of organizations mission, vision, and core values.
Adhere to OSHA standards in delivery of care to all patients.
Perform other duties as directed.
What you will bring to Howard Brown Health:
QUALIFICATIONS, KNOWLEDGE, AND SKILLS REQUIRED:
High School Diploma or GED
Good interpersonal skills and ability to work well within a team setting.
Must be willing to travel at least 50-75% of the time with the providers
Preferred:
Previous experience in a health care setting dealing with HIV/AIDS clients and written or communication skills in English and Spanish.
Medical Assistant Certification
Displays initiative in the work setting
Must demonstrate exceptional written and verbal communication skills
Previous experience working in sexual reproductive health either in a clinical setting or public health setting
