Overview REMOTE - WORK FROM HOME This Engagement Specialist position offers a unique opportunity to engage and educate patients and providers to be a part of Contessa's new Palliative Care at Home program. This position is critical in enabling access to more patients and caregivers to substantially improve the care of patients with serious illness. RATE - $20 - $24 Expected working hours will be 8 AM - 5 PM, Monday

  • Friday. The position is ideal for a candidate who has the passion to help bridge the gap between patients and palliative care education.

    The ideal candidate has demonstrated experience and success building and maintaining relationships to improve patients' access to care. A candidate with healthcare experience, specifically in healthcare community outreach, navigation, engagement, or direct-to-consumer telephonic sales is well suited for this position. Responsibilities We are seeking a dynamic and enthusiastic Engagement Specialist to increase patient awareness and access to Palliative Care at Home.

    The Specialist will help develop, grow, and sustain Palliative Care at Home by executing Contessa's engagement process for every eligible patient and provider. Specialist responsibilities will include: Being one of the patient's first point of contact with Palliative Care at Home Schedule initial visit with patient and/or patient's caregiver Collect information on a patient to help determine patient's eligibility for the program Quickly develop a relationship with the patient and/or their caregiver to build and maintain trust Develop an understanding of the patient's and caregiver's health literacy Demonstrate empathy when interacting with a patient and/or caregiver Make outbound calls to patients and caregivers to educate about Palliative Care at Home and its benefits Enroll eligible patients Make outbound calls to providers of eligible patients to educate about Palliative Care at Home and its benefits Collaborate with providers to receive referrals for Palliative Care at Home and enroll eligible patients Communicate and collaborate with clinicians daily to help identify prospective patients Document patient information in an Electronic Medical Record to assist providers with information that would be helpful for them to provide a better patient experience Resolve patient and provider questions with professionalism and accuracy Demonstrate attention to detail when inputting data into a patient's profile Send communication to patients and providers via Electronic Medical Record, instant messages, email, phone, and mail Qualifications Education High School Diploma or GED required Bachelor's degree (preferred) or 3-5 years of comparable professional experience, ideally in a healthcare community outreach, navigation, engagement, or direct-to-consumer telephonic sales setting Essential Skills / Experience At least 2 years of experience working in a healthcare setting, preferably in palliative care, chronic care, and/or hospice Familiarity with working in an Electronical Medical Record Excellent communication, flexibility, resiliency, and teamwork skills Strong interest in and commitment to delivering home-based palliative care Desirable Skills / Experience Background communicating and corresponding with clinicians Familiarity with medical terminology Telephonic community outreach, navigation, engagement, or direct-to-consumer telephonic sales Capacity to handle difficult conversations Time-management, organization, and accountability Ability to work efficiently in a fast-paced environment while being remote Adaptable critical thinker who can make timely informed decisions Ability to connect with people quickly and deeply to develop a trusting relationship Ability to ask difficult questions to gain a deeper understanding of the patient and the patient's caregiver(s) Bilingual is a plus (English and either Spanish or Chinese preferred) Expected working hours will be 8 AM - 5 PM, Monday
