Hope Network Neuro Rehabilitation serves individuals who have sustained a brain or spinal cord injury caused by auto accidents, falls, gunshot wounds, assaults and more. Hope Network Neuro Rehabilitation focuses on re-establishing work, home and other life activities through intensive rehabilitation programs proven to increase participation and independence after an injury.
The Residential Rehabilitation Technician II is a key associate at Hope Network whose primary responsibilities include:
Responsible for skilled provision of prescribed/therapist directed clinical intervention techniques to patients in the rehabilitation setting to facilitate patient/resident practice and increase safe and independent functioning in individual goal areas, including: mobility, activities of daily living, communication, cognition, health management, psychosocial, leisure and community integration. Demonstrate and mentor new Rehab.
I staff in care and therapy provision and in the use of effective therapeutic working relationships with residents and their support network. Monitor and provide assistance with personal care, protection and supervision, as needed to residents that is consistent with the mission, vision and values of Hope Network.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: This is not intended to be an exhaustive listing of job functions. 1.Regular and predictable attendance is an essential requirement of this position.
2.As a shift leader, teach, mentor and support Rehab. Tech I staff understanding, skill and follow-through on resident Care Plans, medication and medical treatment provision and resident support for personal care, supervision and protection.
3.Supervise directly or track whereabouts of patients depending on the assessed level of independence.
4.Monitor visitors to facility utilizing individual patient's Visitor Guidelines. Communicate details of visits, both positive and problematic to the individual's Social Worker. Forward questions and concerns of members of the individual's support network to the appropriate team members.
5.Report signs and symptoms of health problems by interviewing patients and taking vital signs. Document utilizing a Health Care Assessment form, Seizure Log or other health record.
6.Manage patients' chronic medical conditions, as prescribed and according to training protocols, including Ex. diabetes through application of prescribed diets, administration of insulin and measurement and response to high and low BSLs. Respond proactively to health crises.
7.Escort patients to physician and other community medical and therapy appointments. Relay pertinent information from the treatment team to the medical provider. Communicate and document physician instructions back to the HNNR team utilizing a Health Care Consultation Form. Submit billing information regarding transportation and supervision time according to corporate guidelines.
8.Review and demonstrate knowledge of individual patients' physical/functional and cognitive profile, utilizing strength areas to optimize communication, memory and learning. Reference individual Care Plans to match support and intervention techniques to current function.
9.Provide therapeutic interventions and treatments as prescribed or according the objectives established by patient's therapy team. Utilize adaptive equipment and apply orthotic devices according to physician/therapist instruction.
10. Implement patients' Behavioral Intervention Plans. Document on data collection tools, including sleep logs and complete behavior scales and other standardized measures, as provided by the individual's psychologist.
11. Escort patients to community leisure activities, Ex. sports activities, grocery or personal needs shopping, family events. Provide cues and support for optimal independent functioning and psychosocial relationship skills. Participate and assist individuals with in-house activities to promote meaningful use of free time and offer functional growth opportunities. Record activity participation in individual case record Progress Notes.
12. Respond/follow up on the concerns/questions of the staff; promote positive morale, act as moderator in situations of staff dispute or disagreement involving the workplace.
13. Provide regular resident advocacy and education. May conduct Resident Council meetings and complete action related to feedback gleaned.
14. Others duties as assigned.
Educational / Talent Requirements:
1.Demonstrated ability to communicate in verbal and written formats sufficient to communicate detailed information, critical incidents, physicians' instructions, as well as patient/resident participation and response to clinical interventions.2. Six months experience as an HNNR Rehab. Tech I or two years of experience in a human service agency.3. Demonstrated competency in all HNNR Core Medical Rehab Tech Skills.4. Computer skills including use of Microsoft Office. Ability to learn and use electronic Medication Records, electronic Medical Records and others as needed.5. Certification or ability to obtain Certification in First Aide/CPR.6. Valid Driver's license with driving record acceptable according to Hope Network policy.7. Ability to work independently with flexible hours.
Work Experience Requirements:
1.Six months experience as an HNNR Rehab. Tech I or two years of experience in a human service agency and have completed all HNNR Core and Medical Rehab Tech Skills.
2.Work experience in a licensed residential setting preferred.
