Interviews patients, writes activity assessments and treatment plans for newly admitted patients, serves as liaison for assigned treatment team. Conducts PSR (psychosocial rehabilitation) skills training groups for assigned patients, including geriatric (65+), adults, Intellectually Disabled/Mentally Ill, COPSD (co-occurring psychiatric-substance disorder) and forensic populations; conducts independent studies.

Writes progress notes to report patient response to treatment and completes other documentation as assigned. Serves on departmental committees as assigned. Responsible for cleanliness of work area and equipment.

Provides weekend/holiday coverage as assigned. Provides other duties as assigned by supervisor. Works under the direct supervision of the Program Supervisor and the general supervision of the Program Director.

Essential Job Functions:
1.Completes all interviews and documentation required for Functional Skills and COPSD (co-occurring psychiatric-substance disorder) assessment within the required time frame.
2.Attends staffings, develops recovery plans, documents progress of clients and completes all required documentation within the designated timeframe.
3.Conducts PSR (psychosocial rehabilitation) classes and completes appropriate documentation on clients.
4.Completes other duties as assigned.

Knowledge Skills Abilities:

Knowledge of PSR assessment, teaching techniques/tools, specialty populations, and rehabilitation services.


Excellent written and verbal communication skills. Registration or Licensure

Requirements:

Minimum high school graduation or GED. Bachelor's Degree preferred. Initial Selection Criteria:

Preferred two years' experience in working with specialty populations. (IDD: persons with intellectual/developmental delay; MI: persons with mental illness; persons with COPSD: co-occurring psychiatric-substance disorder; forensic population; etc.)

Preferred six months teaching experience.

Valid driver's license.

Ability/willingness to work on-call weekends and holidays.

Education may substitute for required experience.

Bachelor's degree in associated field is preferred.
