Summary:

Under the direction of the Director of Rehabilitation Services performs off-site preadmission screenings promotes the services of Lifespans inpatient rehabilitation facilities and facilitates appropriate admission to the departments.

Responsibilities:

Demonstrates understanding of Hospitals Mission Vision and Values.

Demonstrates understanding of job description performance expectations and competency assessment plan.

Complies with department and hospital policies and procedures.

Marketing

Promotes the services of Lifespan rehabilitation programs to referring facilities discharge planners social service agencies physicians and third party payers.

Provides educational programs for referral sources

Conducts/arranges tours of center for referral sources/family members.

Pre-Admission Assessment

Considers age specific needs according to scope of services when planning implementing and evaluating care

Identifies and demonstrates ability to assess patients status and interpret the appropriate rehabilitation and medical needs based on age specific or other special needs.

Screens all referrals for appropriate placement based on Medicare and other third party payers guidelines and on the admission criteria established by Lifespan rehabilitation leadership.

Financial

Demonstrates knowledge of current rules and regulations related to third party reimbursement and their impact on rehabilitation admissions.

Confers with financial counselors on pre-admission financial screening.

Obtains pre-certification for admission from third party payers

Communication

Communicates effectively with patients families physicians and other healthcare providers.

Communicates status of pre-admission assessments with medical directors and coordinates admissions with rehabilitation case managers or nurse managers.

Resolves problems and concerns by serving as a liaison between referral sources and Lifespan rehabilitation staff to improve service to the patient and the referring facility.

Communicates patient update and discharge status to the referring individual in a timely manner.

Informs potential and accepted patients and their caregivers about Vanderbilt Rehabilitation Centers program and gathers and responds to input from them regarding their need for rehab information.

Informs Lifespan rehabilitation leadership of issues and/or concerns.

Other Key Functions

Provides demographic data and referral statistics on screening information to Lifespan rehabilitation leadership for trend analysis.

Demonstrates knowledge of UDS and IRF-PAI.

Displays competency in FIM scoring.

Other duties as assigned.

Other information:

Qualifications:

Graduate from a school of nursing and licensed to practice as a Registered Nurse in the State of Rhode Island or in the nurses primary state of legal residency if that state is a member of the Nurse Licensure Compact.

BSN and certification in rehabilitation nursing desired.

Alternate credentials include license to practice Occupational Therapy Physical Therapy or Speech Language Pathology in Rhode Island.

Knowledge of utilization review process health care reimbursement and physical rehabilitation principles.

Current CPR certification.

Must be able to sit or stand 3 -7 hours per day with infrequent twisting and bending.

Must be able to use both hands/wrists perform occasional work that deviates from neutral position and be able to perform fine motor skills.

Must have accurate near and far vision 20/20 depth perception and discriminate among colors.

Must be able to hear whispers at 20 feet.

Assess plan and implement goals for patients with cardiovascular and pulmonary disease within the Cardiopulmonary Rehabilitation program. Assists the Cardiopulmonary Rehabilitation Coordinator in developing and revising emergency policies and procedures Provides safe environment for patients employees students and visitors: * conducting routine checks of equipment * practicing aseptic techniques and infection control Performs comprehensive evaluation and physical assessment of new patients obtains patient history pertinent cardiac and pulmonary diagnostic data and risk factor information from the medical records. Reviews assessments made by exercise physiologist when further evaluation is needed. KEY FUNCTIONS: Prescribes exercise for patients in Cardiac and Pulmonary Rehabilitation and Cardio-pulmonary Maintenance Programs. May collaborate with exercise physiologist to ensure appropriateness of each prescription. Diverse patient populations include post-myocardial infarction post coronary angioplasty stent heart failure heart transplant stable angina diabetes weight management peripheral vascular disease and patients with chronic obstructive pulmonary disease. * Modifies exercise prescriptions when changes in medical status medications or environmental conditions occur. Acts as a case manager to Cardiac & Pulmonary patients; develops individual treatment plans and coordinates the comprehensive multidisciplinary patient care plan with periodic 30 day reassessments. Assesses psychosocial adjustment to illness including depression return to work family adjustments or any significant social dysfunctions that may affect a full rehabilitation. Makes referral to or works with the behavioral therapist on patient�s behalf. Follows routine standing orders for the Cardiac Rehabilitation Pulmonary Rehabilitation and Cardio-pulmonary Maintenance which include the initiation of Advanced Cardiac Life Support procedures ie. Cardioversion/Defibrillation as allowed by their license and training. Receives and transcribes physician�s orders. Provides patient progress reports and maintains correspondence with referring physicians. * Notifies referring physicians of any untoward events. Intervenes when necessary in situations requiring nursing assessment and/or intervention. * Initiates emergency procedures per protocols. * Together with the covering physician responds to all emergencies during the program hours & prepares the patient for transport to the next level of care. Recognizes absolute and relative contraindications for exercise and enrollment as outlined in ACSM and AACVPR guidelines Follows AACVPR guidelines pertaining to documentation with the patients treatment plan. Responsible for tracking and measuring patient outcomes using department database and national registry. Familiar and knowledgeable regarding the 10 core components of cardiac rehab and the 12 core components of pulmonary Rehab Assists co-workers in the interpretation and significance of any electrocardiograph or physiological changes that the patient may be experiencing. * Serves as a resource for the other professionals who work closely with the Cardiac Rehabilitation population. KEY FUNCTIONS: Provides on going patient education as it relates to cardiac and pulmonary disease management. Conducts patients� education classes which include ongoing revision and evaluation of materials. May assist manager in the functional supervision and instruction of the nursing and exercise physiologist students and volunteers. May perform routine graded exercise testing on the cardiac rehabilitation and maintenance patients. Knowledge of ACSM/AACVPR guidelines for contraindication of stress testing and endpoints to stress testing. Assists with maintenance of emergency supplies and crash cart. Participates in hospital nursing committees and projects that promote the growth and development of the Cardiac and Pulmonary Rehabilitation Program Provides in-service training to professionals interested in cardiac rehabilitation and exercise physiology. Maintains quality assurance safety environmental and infection control in accordance with established hospital department policies procedures and objectives Demonstrates ability to: * monitor patients for contraindications of exercise * document blood pressure heart rate pulse oximetry rhythm symptoms and ECG * observe patient for abnormal exercise respons0es * respond to patient emergencies Upholds the organization�s standards of nursing practice as defined by the Nursing Division.
