What You Will Do
As an Associate with Ascension-WI, you will have the opportunity to evaluate and treat patients with physical, emotional, cognitive and/or psychosocial deficits which impact their abilities to successfully perform independent living skills and assisting patients with return to work.
Interviews patient to determine medical history, condition, desired results and appropriate modality.
Assesses quality of patient care and coordinates patient care services with patients, staff, physicians, and other departments.
Educates and counsels patients, caregivers and family members on treatment options, placement in alternative care settings, and continuing care.
Documents patient information, interview responses, treatments, feedbacks and results.
What You Will Need
High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required
Master's degree in Vocational rehabilitation counseling preferred
