The Rehab Supervisor is responsible for assisting in coordination and management of campus PT, OT, ST offerings to LTC campus residents. The Supervisor provides additional management, leadership, and oversight of the Therapy Department. This position primarily oversees quality therapy services to skilled nursing facility residents, mentors therapy team and fosters their growth and skills sets, facilitates SNF discharge planning in collaboration with other involved disciplines (i.e. nursing, social work, dietary, spiritual services, Life Enrichment, Assisted Living, Independent Living, etc) based on individual resident need, maintains data for QA and adjustment in treatment as identified through reflection of client and industry tendencies. Provides back-up for Director of Rehab.

This position is accountable to the Director of Therapy & Fitness.

ESSENTIAL RESPONSIBILITIES (including but not limited to)

Coordinate & maintain SNF therapy service alignment and interdisciplinary work with nursing, social work, dietary, and Life Enrichment.

  • Mentor and guide Therapists including Physical Therapists, Physical Therapist Assistants, Occupational Therapists, Occupational Therapy Assistants, and Speech-Language Pathologists. Assist in establishing employee objectives and standards. Establish work priorities, assignments, and activities. Assist in performance evaluations.

  • Actively participant in therapist annual performance reviews alongside Director of Therapy & Fitness. Actively participate in any therapist performance improvement plans alongside Director of Therapy & Fitness as appropriate.

  • Develop and maintain rapport with other department personnel to ensure effective exchange of information regarding residents for care delivery, discharge planning, facility operations implementation and success to amplify collaborative approach.

  • Responsible for informing therapist team of imminent changes in discharge planning as information is gathered from interdisciplinary team to ensure effective utilization, facilitation to process, and data collection accuracy.

  • Responsible for continuous data collection of the SNF subacute rehab residents to then analyze and reflect on trends and tendencies impacting census, quality service delivery, and outcomes alongside Director for strategic planning based on statistics indicating success or indicated change. Submits daily department utilization to Billing for daily snapshot contribution.

  • Assists in SNF resident regulatory administrative tasks such as completing Admission and Discharge UDAs, upkeep of discharge records for QAPI and trend reflection, closure of billing records with accuracy to resident census.

  • Acts as administrative sustainability for leadership of department including assistance in payroll/work hours distribution, decision making for operations and support to staff across campuses as well as to counterpart, attends essential meetings for management of service delivery opportunities and presence to all household level staff and support staff, and directs problem solving and department planning in the absence of Director.

  • Facilitates regular caseload meetings for proactive and coordinated care and discharge planning of

SNF residents. Provides summarized information to interdisciplinary team for Medicare compliance and facility census management. Attends SNF PDR meetings for support to IDT in psychotropic drug use and appropriateness by contributing to overall presentation and wellbeing of residents discussed.

  • Provides backup to administrative tasks including insurance reviews, therapy service line scheduling, DME acquisition processes, care conference coordination for therapist attendance, insurance verification requests, and order processing with support of Director in absence of Administrative Assistant.

  • Open to facilitating and supporting professional students and their clinical instructors during clinical internships for completion of certification, licensing, and program completion.

ESSENTIAL QUALIFICATIONS

  • Ability to supervise and support Physical Therapists, Physical Therapist Assistants, Occupational Therapists, Occupational Therapy Assistants, and Speech-Language Pathologists.

  • Ability to provide quality services to residents, customers, and other clients.

  • Excellent communication/listening skills including ability to relate well to the aging population.

  • Strong organization and task execution skills.

  • Ability to work independently.

  • Ability to maintain and protect the confidentiality of information.

  • Ability to exercise good judgment and make sound decisions.

  • Ability to work in a partnership environment which fosters effective teamwork in meeting the mission of Oakwood. Willing to travel between Oakwood campuses to maximize staff utilization and prioritize needs of resident care.

  • Ability to detect change in mental or physical conditions of residents and communicate changes to appropriate staff members and support other staff who experience difficult resident interactions and/or situations.

  • Ability to adhere to an efficient schedule on a continuous basis.

  • Ability to coordinate and supervise planned exercise programs for varying physical abilities and adapt tasks as necessary to accommodate resident limitations. Focus is on active aging and the recognition of modification needs of the aging body.

  • Ability to develop personal training assessments and programs and offer instruction/supervision as needed.

EXPERIENCE, EDUCATION, AND/OR TRAINING

  • Graduate of an accredited PT, PTA, OT/R, COTA, or SLP Program.

  • Current and active Wisconsin PT, PTA, OT/R, COTA, or SLP license required.

  • Demonstration of leadership qualities and eagerness to learn and be responsible for business aspects of SNF and other levels of therapy service lines desired.

  • 3-5 years experience as a licensed therapist in SNF setting required.
