Overview

Seeking Occupational Therapist or Physical Therapist to serve as Rehabilitation Manager at our Assisted Living facility located in Johns Creek, Georgia

The primary purpose of the position is the management and administration of all aspects of rehabilitation services for the respective communities as well as for maintaining a clinical caseload.

The Rehabilitation Manager serves as an advocate, educator, liaison and mentor to interdisciplinary team members to include but not limited to community staff, home health staff, hospice staff, physicians, nurses and others.

Responsibilities

  • Demonstrates strong clinical knowledge and understands the role of rehabilitation for all clinical disciplines and product lines within the Assisted Living, Independent Living and CCRC settings.

  • Represents HealthPRO-Heritage in a professional, courteous and competent manner as the point of contact for all community customer concerns or issues and problem resolution.

  • Acts as a liaison between HealthPRO-Heritage management, Home Health, Hospice and community administration and is able to communicate assertively, diplomatically, and clearly through informal and formal mechanisms.

  • Participates in the implementation of Quality Improvement Programs.

  • Participates in patient care meetings to facilitate open communication while acting within HIPAA guidelines.

  • Provides classes and education to the community, physicians, other healthcare providers as well as community staff, residents and their families regarding therapy and facility services.

  • Participates in community events that describe therapy services.

  • Promotes positive employee relations by facilitating an atmosphere of open communication and problem solving through scheduled team meetings that adhere to all regulatory requirements.

  • Develops special programs for resident participation, such as fitness, dementia programming, and various protocols for activities that strengthen and enhance the existing therapy program.

  • Provides leadership to and directly manage clinical therapy staff, facilitating a team atmosphere among staff.

  • Supervises Rehabilitation Technicians, students and volunteers as needed.

  • Reviews the quality and appropriateness of the total services delivered and individual therapy programs for effectiveness and efficiency, using pre-determined criteria.

  • Contributes to a positive work team by sharing information, using problem-solving methods, accepting new ideas, constructive feedback and advice from others.

Qualifications

  • Bachelor of Arts or Science Degree or a Master's Degree.

  • Assumes responsibility for ongoing continuing education and professional development

  • Must maintain valid state professional licensure

PRIMARY RESPONSIBILITY Support the Executive Director of Accountability and Assessment with executive level leadership, direction, and continuous quality improvement of an effective student accountability support program which accomplishes the mission and strategic vision established by the Board of Education and the CEO/Superintendent. ESSENTIAL JOB DUTIES 1. Support the Executive Director of Accountability and Assessment with the development, implementation, evaluation, and continuous improvement of the balanced assessment program. a. Support the Executive Director in assuring the statewide K-12 assessment program is implemented and monitored for compliance at the school and system level. b. Support the Executive Director of Accountability and Assessment with the planning, implementation, and continuous improvement of the GCPS balanced assessment program to assure the reliability, validity, and policy implications are appropriate. c. Support the effective administrations of local, state, national, and international assessments. d. Support with the reporting student, teacher, school, and system-level results including analysis as appropriate. e. Support with recommendations of district procedures for assessment to assure security, administration, and appropriate reporting of results and student data. f. Support the Executive Director with the communication of accountability and assessment information to the Board, CEO/Superintendent, Cabinet, Leadership Development, and other audiences as needed. 2. Support the Executive Director in the development and implementation of the Student Performance Goals (SPG) and their accompanying assessments. 3. Support the timely completion of College and Career Readiness Performance Index (CCRPI) data collection, submission and validation, meeting all deadlines and requirements from the Georgia Department of Education. a. Assist with providing leadership training in analysis of student achievement data to inform instruction. b. Assist with training on the effective development and use of common assessments. c. Support cross divisional data quality processes, analysis, and reporting of student achievement data. 4. Assist with the development of innovative tools of assessment for learning, e.g., Gateway and Curriculum Embedded Performance Tasks. 5. Assist with the effective planning, implementation, and monitoring of budgets to support primary functions of the Offices of Accountability and Assessment. 6. Assist with the development of professional learning for program implementation and improved student achievement using innovative delivery models which focus on the building of capacity and sustainability for schools. 7. Communicate effectively with all internal and external audiences as needed to support the district's vision, mission, and goals. 8. Perform other duties as assigned.
