Cardiothoracic PCU is a 36-bed unit with a patient ratio of 1:3. This unit will care for patients with Cardiac or Thoracic needs, surgical related, and or overflow from any of our Cardiology sister units. You will get to learn side by side with our already established caring specialized trained Cardiology units.
Cardiopulmonary PCU is a 40-bed unit with a patient ratio of 1:3. This unit cares for patients with cardiac and pulmonary needs such as pre/post heart catheterizations, pacemaker insertions, ablations, lead revisions, tikosyn and sotalol initiation, rapid atrial fibrillations, etc.
Cardiovascular PCU is a 4-bed unit with a patient ratio of 1:3. This unit cares for patients recovering from Cardiovascular and Thoracic surgical procedures which includes, coronary bypass grafting, valve repair and replacement, Transcatheter Aortic Valve Replacement (TAVR), Pericardial window, device explanation and laser lead extraction, and all Thoracic surgeries including esophagogastrostomy, lung resection, as well as thoracic and abdominal aneurysm repairs. Cardiac PCU is a 40-bed unit with a patient ratio of 1:3. This unit cares for patients from our Advanced Cardiology Surgical ICU ECMO unit. We care for advanced heart failure therapies and is unique since we work with pre-and post-Left Ventricular Assist Device (LVADs) artificial hearts, and heart transplant recipients. We also care for general cardiac patients often prep patients for bypass and valve repair, plus pacemakers, AICDs, and heart catheterizations.

You Will Be Responsible For:

NURSING PROCESS: Completes timely assessments per Practice Guidelines including physiological, psychological, developmental, sociocultural, spiritual, and life-style factors, including signs of abuse or neglect. Makes appropriate decisions and interventions and nursing diagnosis according to patients actual or potential health conditions or needs. (American Nurses Association [ANA], 2012)
NURSING PROCESS: Based on the assessment and diagnosis, sets measurable and achievable short and long-range goals for the patient (ANA, 2012), assisting in the development and implementation of an individualized plan of care. Prioritizes and completes follow up assessments, evaluates and modifies plan of care as needed. Nursing practice reflects mutual goal setting and whole person care including meeting the needs of the mind, body and spirit. (King, 1995)
NURSING KNOWLEDGE: Demonstrates the ability to assess and interpret diagnostic data relative to patient age and condition, including, but not limited to lab results, non-invasive monitoring data and interdisciplinary team notes. Nursing practice reflects the application of knowledge and skills pertaining to all diagnoses, procedures, medications, complications, and equipment associated with unit patient population.
PATIENT SAFETY: Nursing practice reflects the application of knowledge, skill and adherence to all pertinent regulatory standards. Follows strict infection prevention precautions. Adheres to policy and procedure regarding all aspects of medication safety. Practice reflects importance of patient teaching and takes advantage of opportunities, both planned and unplanned, to carry it out in accordance with plan of care.
COMMUNICATION: Promotes an exceptional patient experience by abiding to compassion and image standards and fostering excellent nurse-patient relationships through effective/therapeutic communication with the patient, family, and visitors, including but not limited to and as applicable, hourly rounding. Exhibits ability to utilize appropriate techniques, verbiage, and resources in all interactions with the patient to their level of understanding. Communicates effectively with interdisciplinary team by completing thorough hand off communication in all situations. Follows chain of command when appropriate.

Qualifications

What Will You Need:

EDUCATION AND EXPERIENCE REQUIRED:

Graduate of a school of nursing

Education and Experience Preferred:

Bachelor's degree in Nursing

LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:

Current valid State of Florida or multi state license as a Registered Nurse
Basic Life Support (BLS) certification
Advanced life support certification (ACLS, PALS, NRP) per specialty/unit requirements.

Licensure, Certification, or Registration Preferred:

Professional certification
ACLS, PALS, NRP (based on patient population; See document EKG and Advanced Life Support Requirements)

Job Summary:

The Registered Professional Nurse (RN) is responsible for providing and supervising direct and indirect total nursing care responsibilities to identified age specific groups. Utilizes the nursing process (assessing, planning, implementing and evaluating) in achieving the goals of the nursing department. Adheres to the AdventHealth Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies and accrediting bodies. VIEW JOBS LOCATION: Pruitthealth Hospice Columbia JOB PURPOSE: Screen referral for appropriateness and perform admission to facilitate the transition of the patient and family into the hospice program. Also responsible to Case Manage patients and families to ensure symptom management, patient and family education, medication management, coordination of hospice services and provide excellent communication with the patient's physician. KEY RESPONSIBILITIES: * Coordinates the interdisciplinary care team for assigned patients, and is designated to coordinate the implementation of the plan of care. * Provides the primary nursing care for hospice patients, regardless of the setting. * Assumes full responsibility for professional management of assigned hospice patients' care related to the patient's terminal illness. * Teaches the primary caregiver skills that are needed to care for the patient in their residence. * Works closely with the attending physician and other team members to develop appropriate pain and symptom management. * Completes nursing assessments and admissions of patients. * Maintains the clinical record to meet federal, state, and agency regulations and guidelines. * Supervises hospice aides and certified nurse aides, making a supervisory visit at least every 14 days, and face to face at least monthly. * Assumes all on-call responsibilities as assigned. * Participates in Performance Improvement Program. * Participates in and presents inservice programs, team conferences, and staff meetings. * Provides skilled nursing care to assigned patients/families as outlined in the plan of care. * Participates in the development of the patient/family interdisciplinary care plan. * For patients residing in a skilled nursing facility, the RN Case Manager will have responsibility for managing the patients' symptoms related to the terminal illness. * Verify by patients visit and review of documentation that the caregivers in the SNF are carrying out the plan of care as written. * Have ultimate responsibility for developing and integrating the plan of care with the hospice team and the SNF team to assure one integrated plan of care that reflects the hospice philosophy. * For patients receiving private home care services, the RN Case manager shall supervise the certified nursing aide, sitters, and companion services assigned to patients. * Perform other duties as assigned by the DHS or the Administrator.
