Our Endocrinology Office is looking for a Full Time Certified Diabetes Educator/ RN to join their team!

This is a 40 hour week. The hours are: 7:30-4pm

Responsibilities:

Provide personalized diabetes self-management education to individuals and groups according to scope of practice, standards of practice, and Standards of Professional Performance for Certified Diabetes Educators.

  • Works as a member of the multidisciplinary healthcare team in implementing a comprehensive diabetes self-management education (DMSE) program, including planning and oversight of educational interventions.

  • Participates in the diabetes education program by teaching self-management strategies to persons with diabetes, their families and caregivers.

  • Provides individualized assessment, plan and follow-up evaluation encompassing one-on-one and group sessions for patients in the program, utilizing appropriate methods to deliver educational information.

  • Works collaboratively with all health care providers referring patients to program.

  • Obtains regular continuing education relevant to diabetes management.

  • Implements quality practice by following applicable policies, procedures, legislation, requirements of licensure, as well as Standards of Practice and Standards of Professional Performance for Certified Diabetes Educators.

  • Evaluates processes and outcomes.

  • Communicates sound, scientific principles, research and theory to clients and healthcare providers and helps them apply knowledge and skills to diabetes self-management.

  • Documents relevant information in the patient's electronic medical record.

  • Maintain strict patient confidentiality pursuant to HIPAA and NYS Public Health Law

  • Report incidents of observed or suspected non-compliance with universal precautions to the Director.

  • Responds appropriately to incidents of exposure to blood-borne pathogens.

  • Travel to office locations.

Annual Salary Range: $59,411.50- $80,205.50
