The Registered Nurse-Wellness Coach-Community develops and implements the adolescent behavioral health home to include: identifying individuals who have extensive co-morbid physical and behavioral health conditions, completes yearly physical assessments, develops wellness plans, communicates regularly with individual consumers, families, caregivers, county representatives and local health care providers to establish priorities, identify training opportunities, and identity problem areas/needs.Duties may include and are not limited to:*
  • In cooperation with assigned case manager, identifies high-risk youth and/or families related to wellness levels, especially in the physical dimensions, using approved physical health assessment(s).*
  • Develops wellness plans in collaboration with the assigned case manager, the youth and the family.*
  • Implements wellness plans with focus on motivating and coaching all involved, and following up on progress. Uses the CCBH web portal to monitor progress for each participating individual. Assists with the use of recovery and wellness tools to support independence.*
  • In collaboration with the blended case managers will complete an integrated health assessment for individuals fourteen and older one time per year in order to develop wellness goals.*
  • Outreaches with other medical professionals (primary care physicians, specialists, etc.) and documents accordingly in order to review the integrated health screening as well as to receive progress updates from PCP.*
  • Consults with the blended case managers in order to review individual's progress towards wellness goals and uses the data to identify trends and, when necessary, generate recommendations for improvement.*
  • Stays current with practices related to all aspects of wellness and educates others to include youth, families, case managers and/or any other individuals involved in the youth's treatment plans.*
