We are looking for a home health Registered Nurse (RN) to work weekends, covering multiple care centers in Connecticut.

There is some flexibility regarding the schedule (days and times). This could be full-time, part-time, or per diem/PRN.

Coverage areas may include: Stamford, Hamden, Shelton, Danbury, Bridgeport, Ansonia, and Beacon Falls.

What You Will Do:

As a home health nurse with Amedisys, you will provide one on one skilled nursing care and treatment to patients in their home using a comprehensive and patient specific care plan, innovative solutions, and a preferred technology for point of care documentation. You will have the opportunity to spend quality time with the patient, in their own home, to promote their health and independence. You will have autonomy in addition to the support and collaboration of a full team of healthcare professionals.

  • Collaborate with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop a patient specific care and treatment plan

  • Cooperatively utilize varied solutions to educate and promote the health and independence of the patient

  • Assess, monitor, document and report progress of patient's health and condition using required documents, reports and established timelines

  • Provide supervision and evaluation of aides and licensed practical nurses/licensed vocational nurses

Required:

  • Current and unencumbered license to practice as a Registered Nurse specific to that state the employee is assigned to work by the Company.

  • Minimum of one (1) year of experience as a registered nurse

  • Current CPR certification

  • Valid Driver's License and reliable transportation and agency required liability insurance to complete home visits.

Preferred:

  • Bachelor of Science in Nursing (BSN) degree

  • Acute care experience as a registered nurse with the adult population in a medical surgical environment

  • Previous home health or hospice experience

As a member of the Rapid Response Team the resource nurse responds to cardiac and respiratory arrests, all trauma alerts and codes, PAMI's, stroke alerts, as well as provides rapid treatment to deteriorating patients in order to prevent or stop hospital complications from occurring. The resource nurse also facilitates patient throughput within the organization. CORE COMPETENCIES/CRITICAL RESPONSIBILITIES: * Coordinates and delivers patient care utilizing the Nursing Process and following all established standards of practice within State and Federal guidelines. * Responsible for responding to all emergencies (codes; traumas; medical, stroke, vent, and sepsis alerts; and PAMIs) * Available as a critical care resource to nursing staff. * Assists staff nurses with patients experiencing a change in status warranting immediate intervention and potential transfer to critical care areas. * Performs intentional rounding on all nursing units to identify patients at risk for clinical decompensation. * Performs intentional rounding on patients that have been transferred out of the ICU for the first 24 hours. * Collaborates with the nursing staff and MDs for optimal treatment of the ETOH withdrawal population. * Assists in facilitating and improving throughput of patients from the ED. * Resource for nursing staff for difficult peripheral IV insertions. * Resource for nursing staff on restraint use and documentation. * Develops and communicates patient plan of care reflecting patient issues and proposed interventions to achieve desired patient outcome. * Resource for ED and ICU staff on the administration and documentation of tPA. * Collaborates with patient, family and healthcare team in implementing plan of care. * Provides education as indicated after assessing the learning needs of the patient and family. * Participates in and supports performance improvement and risk management activities and risk management activities. * Acts as preceptor and mentor to new nursing staff and nursing students as requested.\ Shift - Part time nights: 7:00pm-8:00am, 30 hours/week MHOSP Required Skills QUALIFICATIONS REQUIRED: 1) Graduate of an accredited School of Nursing. 2) Valid license as a Registered Nurse in Connecticut. 3) BSN highly preferred. 4) Minimum of 2 years experience in a critical care setting (ICU or ED). 5) Maintains a minimum of 6 contact hours annually. 6) Certifications – BLS, ACLS, PALS, TNCC. 7) Experience with IV insertion and conscious sedation highly preferred.
