Overview of Registered Nurse Position:

Responsible for coordinating and providing high quality multidisciplinary home health care services for patients consistent with VNA Care's policies, goals and objectives. Works under moderate supervision.

Education:

Graduate of a certified RN program required; Bachelor of Science Degree in Nursing preferred.

Licensure:

Licensed to practice in Massachusetts.

Experience:

Minimum of one year of recent med/surg experience in an acute care setting preferred; home care experience preferred.

Community Healthlink (CHL) is looking for a per diem RN (4-24 hrs/wk) in Northern Worcester County, MA. The Early Intervention (EI) team provides strength-based, family-centered services for young children under three years of age who have developmental delays or are at risk of a developmental delay. Services are designed to meet the needs of each infant or toddler, and the needs of their family. The EI team provides services in the home/community setting with the goal of enhancing the child's physical, cognitive, communication, social-emotional and adaptive skills. This position offers a wonderful opportunity to join an expanding multi-disciplinary team of passionate professionals who are dedicated to providing the highest quality of care for young children and families. In this role, you will provide direct intervention services to children 0-3 and their families. Responsibilities * Provide direct intervention services to children 0-3 and their families * Conduct evaluations and assessments particular to individual specialty * Develop and implement Individualized Family Service Plans * Coordinate services and act as a resource for children and families * Assist in transition planning prior to discharge from the program * Attend staff meetings, in-service education and other professional meetings as required * Maintain client and administrative records according to center policy * Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors Qualifications * Bachelors Degree * MA RN license required * Experience working with young children and families * Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes * Ability to work independently and contribute to overall success of team * Excellent interpersonal, verbal, written and communication skills; flexibility * Spanish speaking candidates encouraged to apply * Must be able to pass a CORI background check Community Healthlink is an Affirmative Action/ Equal Opportunity Employer.
