Responsibilities

  • The RN My HealtheVet Change Agent Coordinator serves as facility champion, leads facility teams to support existing and future Veteran communication initiatives, and coordinates local implementation, training, communications, outreach, web health content and support efforts.

  • My HealtheVet is a Department of Veterans Affairs 0/A) sponsored website for Veterans, their families, advocates, and VA employees. MHV is a web-based application designed for Veterans and their families with the goal of optimizing Veterans' healthcare.

  • Through MHV, Veterans can build their own Personal Health Record by self-entering personal and health-related information, as well as their health readings.

  • MHV online prescription refill is available to all Veterans receiving medications at Veterans Affairs Medical Centers/Healthcare System (VAMC/HCS), and MHV provides one central location for obtaining information about VA benefits, programs, and services

  • The RN MHV Change Agent Coordinator plans, organizes drives, directs, evaluates, and reports local MIN-related actions/activities.

  • The MHV Change Agent Coordinator leads the facility's efforts to integrate MHV and SM among healthcare providers and patients.

  • Serves as SM liaison and administrator to VISN and National MHV teams.

  • This position is responsible for establishing and managing communications plans that identify target audiences, appropriate messages, activities, and resources needed for effective communication, and effective communications plan for change management, incorporate multiple channels to manage expectations and disseminate project progress.

  • Responsible for supporting and guiding facility VHA public affairs officers and stakeholders about My HealtheVet. Oversees MHV's online presence and web content and leads innovations to keep facilities MHV on the cutting-edge.

  • Ensures accountability via written communication as it relates to MHV as each of these communication skills contributes importantly to the way VHA communicates with Veterans, Congress, our dedicated volunteers, the public and employees.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.

  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience: Two years of experience

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.

  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.

Physical Requirements:
Work is primarily sedentary, but may require periods of standing during presentations, briefings, speeches, health fair activities, etc., although some slight physical effort may be required.

This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The population of male and female Veterans served ranges from the adult, age 18, to the elder adult, age 65, and over. Each Veteran may be accompanied by caregivers/family members, which may include small children. Veterans may present to clinic via wheelchair, scooter, or gurney. This position may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. This position may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The MHV Coordinator must be a mature, flexible, sensible, individual capable of working effectively in stressful situations, able to handle multiple inputs and tasks simultaneously, able to shift priorities based on clinic or patient needs, and readily able to accommodate to change or new situations and realities as doable challenges.

Support's the organization's philosophy of nursing. Ensures the safety and well being of each patient and that the standards of nursing and patient care are met. Essential Function: * Assess physical, psychological and social dimensions of patients, obtain health history, and provide a written assessment of patients upon admission. * During confinement, and upon discharge; document and communicate all triage decisions to appropriate health care provider. * Develop individual nursing care plan using nursing assessment protocols in consultation with health care providers. * According to physician orders implement medical care plans to include diagnostic tests, EKGs, etc. * Administer medications using proper techniques, procedures and approved routes of administration; nutrition and therapeutic diet plans. * Assist physician in medical or minor surgical procedures as necessary. * Provide and monitor direct patient care. * Document nursing encounters utilizing the Problem Oriented Medical Records (POMR) form of charting. * Maintain accountability of all controlled substances and prescription medications according to federal and state laws, and organizational regulations. * Coordinate with medial, dental, mental and ancillary personnel as required to ensure continuity of care. * Follow established policies, procedures, continuous quality improvement and safety, environmental and infection control standards. * Will adhere to Armor's Standards of Professional Conduct, and will incorporate and/or be supportive of Armor's four core FISH!
