Description

Traveling between multiple locations to perform the duties and responsibilities of the role, the Traveling RN is accessible to staff at all locations during working hours. Depending on the program, may be required to be accessible after hours for consultation.

Responsibilities:

Provide clinical care to address the medical needs of individuals in multiple locations within the assigned region.

  • Conduct the medical assessment of each individual referred to the program and collaborate with an Intake staff member to determine appropriate placement in the community

  • Participate in the development and implementation of service plans and supervise, in conjunction with the physicians, the medical components of the plan

  • Serve as liaison and advocate with other medical and professional service providers

  • Provide input regarding health care services to be included in each Individual Service Plan (ISP)

  • Review documents and revise all nursing/nursing related care plans on a regular basis

  • Promote health and wellness for individuals and ensure nursing actions and steps are put in place for prevention

  • Ensure medical records and reports are current and complete in accordance with regulatory and corporate standards, including medication administration records and physician's orders

  • Maintain contact with primary care physicians and specialists, and, when directed, attend doctor visits with mentor and/or individual receiving services.

  • Administer medications according to established guidelines

  • Provide documentation for all medications given and review medication administration records for compliance

  • Oversee the management of high-risk medical conditions and private duty nursing services

  • Provide technical assistance, medical oversight and training of LVNs and program staff to ensure the effective implementation of all related services

  • Develop training flow charts specific to each individual receiving services, provide and document medical skills training to clients, mentors, respite providers, staff, and where appropriate, the biological family

  • Participate as an Individual Health Plan (IHP) or Plan of Care (POC) team member

  • Evaluate the medical needs of each individual for initial IHP/POC,

  • Ensure IHP/POC goals and objectives are medically indicated

  • Present medical skills components at Pre-service Orientation

  • Generate ongoing health care protocols for the specific medical conditions outlined in the IHP/POC

  • Provide consultation and technical assistance to staff and mentors regarding home care issues along with operation of durable medical equipment.

  • Participate in on call system as needed

  • Travel between multiple locations during work hours

  • Provide support and is accessible to staff at all locations during working hours

  • May be required to be accessible after hours for consultation

  • May actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee and Administration meeting

  • Graduate of an accredited RN program

  • One year experience working with medically complex individuals

  • Current state RN License

  • Current driver's license, car registration and auto insurance if driving on the behalf of the Company or providing transportation to individuals receiving services.

  • Current CPR/First Aid certification.

  • Job requires performance of duties that involve potential for exposure to blood, body fluids, or tissues. Tasks that do involve exposure are an expectation of employment.

  • Experience working with individuals with Intellectual or Developmental Disabilities is preferred, but bring the right attitude, and we'll train you for success

  • Excellent assessment skills with the ability to exercise critical thinking in the clinical environment. Ability to prioritize, strong organizational skills

  • Effective communication skills a must ability to communicate clearly with team members, medical professionals, those we serve, and their families

  • Position requires travel in assigned region to provide services and conduct visits to our individuals.

The VITAS RN is designated to provide coordination of care, ensure continuous assessment of each patient's and family's needs and implementation of the interdisciplinary plan of care. Work Schedule: Full-time Weekends, Friday to Monday, 8 AM - 5 PM in Sussex County. Case Management and Supervision * Follows VITAS policies and standards (See VITAS Standards Manual) and practices within the scope of the RN license * Reports any request to practice outside of scope of license to the manager/supervisor * Supervision of (VITAS employed and contracted) nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses (and contracted Registered Nurses) and Volunteers per the plan of care * Completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement * Appropriately identifies/utilizes resources, referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults * Completes documentation at the time of the event and at the location of care unless otherwise directed * Provides education to patients, families, caregivers and referral sources * Participates in patient care meetings as assigned * Participates in weekly team meetings * Provides effective communication/shift reports through written, verbal, reporting and body language * Participates in bereavement support * Participates in the on-call rotation and/or provides shifts during non-business hours such as evenings, weekends and holidays based upon program-identified needs * Provides visits/shifts in program coverage area * Develops, coordinates, and follows the patient's individualized plan of care * Appropriately delegates nursing tasks to Hospice Aides/Homemakers, Volunteers, and Licensed Practical/Vocational Nurses * Connecticut -For activities other than those listed in section 1913D69(d)(3)of the Connecticut Department of Public Health delegated to a Hospice/Homemaker, the RN will document in the clinical record that: * All factors pertinent to the patient's safety, including the competence of the Hospice Aide/Homemaker have been assessed * Determination that this activity can be delegated safely to a Hospice Aide/Homemaker * Provides nursing care including nursing tasks that may be delegated to other roles such as LPNs, HHAs and Volunteers * Participates in and monitors patient and family care compliance activities * Professionally manages/coordinates the hospice plan of care * Provides and follows infection control practices * Delivers care in accordance with the patient's plan of care * Provides primary nursing care for inpatient and continuous care patients as assigned Quality Improvement * Participates in the Quality Assessment and Performance Improvement (QAPI) Program and related activities as assigned.
