JOB SUMMARY

Under the direct supervision of the Supervisor, Nursing Unit, and according to hospital policies and procedures, the RN assesses, plans, implements, and evaluates nursing care for her/his assigned patients.

Perform complete and accurate nursing assessment of assigned patients

Completes admission interview accurately within 24 hours of patient admission.

Performs and legibly documents patient assessment completely and accurately per shift, or more frequently, as indicated by patient condition.

Identifies changes in patient condition (i.e., test results, abnormal findings, etc.) and reports to the physician and/or Supervisor, Nursing Unit.

Assesses and legibly documents patient educational needs on admission and throughout hospitalization.

Plans comprehensive nursing care for assigned patients according to written hospital standards and physician orders.

Initiates and individualizes nursing care plan within eight hours of admission.

Identifies and prioritizes relevant nursing diagnoses.

Reviews and updates nursing care plan every 24 hours.

Initiates discharge planning upon admission.

Provides and legibly documents skilled care to assigned patients in compliance with written hospital procedures and according to physician's orders.

Performs and legibly documents nursing care according to patient needs in a competent and timely manner.

Accurately administers and documents medications within 30 minutes of indicated time or as patient condition warrants.

Provides individualized nursing interventions based on identified nursing diagnoses and plan of care.

Carries out appropriate interventions based on identified changes in patient condition.

Evaluates patient care outcomes in an appropriate and timely manner.

Evaluates plan of care to determine need for modifications in a timely and prompt manner.

Legibly documents resolution of patient problems identified in plan of care.

Evaluates effectiveness of patient teaching and revises teaching plan as indicated.

Participates in Quality Improvement activities.

Maintains current professional nursing knowledge.

Legibly documents evidence of a minimum of six hours of in-house staff development, in addition to mandatory education requirements, or continuing education relevant to the clinical practice area.

Completes unit specific competency check list and age specific competency requirements.

Education/Experience

Required: Graduation from a school of professional nursing. At least 1 year of recent experience as an Staff RN in an acute care setting.

Preferred: Completion of or actively pursuing a BSN degree.

License/Certification

Required: Current license to practice as a Registered Nurse in Texas. If Compact Licensed, Texas License must be obtained within 90 days of hire.

Current certification from the American Heart Association as a BLS provider. ACLS certification required within 90 days of hire.

St. David's South Austin Medical Center is an acute care facility with 334 beds, offering a range of complex specialties and sub-specialties, including a nationally accredited oncology program with the area's only adult bone marrow transplant program; a trauma program that includes all of the capabilities and programmatic elements of a Level II trauma center to treat the most severely injured and critical patients; a comprehensive cardiac program; full-service maternity and newborn care with Level I and II nurseries; and two full-service emergency centers in the communities of Bee Cave and Bastrop. 4 Central is a 36 bed Medical/Surgical Telemetry unit. This unit also houses the post cardiac cath patients. The patient population consists of adults and geriatric patients with a wide variety of diagnoses. This unit provides care to telemetry patients. Position Description: The Registered Nurse (RN) utilizes the nursing process to direct and deliver comprehensive nursing care to patients on an assigned unit in a safe and proficient manner according to individual patient needs. Provides direct patient care based upon South Austin Medical Center Hospital Standards of Care and Standards of Practice, Polices, Procedures and Practice Guidelines. Provides guidance and supervision to Licensed Vocational Nurses (LVN), Nursing Technicians, Nursing Assistants and Unit Secretaries. He/she acts as patient and advocate with the Hospital environment. Assumes Charge Nurse responsibilities and accepts assignments on other units when necessary. Principal Responsibilities: * Considers age related factors in the assessment, care, and treatment of pediatric patients. * Considers principles of death and dying when caring for terminally ill oncology patients. * Provides emotional, spiritual, religious and cultural support to patients and their significant others * Applies principles of orthopedic nursing. He/she is familiar with various types of tractions, casts, splints and with various orthopedic surgeries. * Applies principles of urological nursing when caring for that type of patient. * He/she is familiar with various urological catheters and various urological surgical procedures. * Knows how to take care and maintain central venous access catheters. * Demonstrate understanding of HCA's and "Patients First " safety initiative by strict compliance to all safety protocols and procedures.
