Texas Health Action (THA) is currently seeking Registered Nurses at our San Antonio Kind Clinic. This role will report to the Clinic Administrator with a clinical reporting relationship to the Medical Director.

What you would do:

  • Assist with the evaluation and care of patients seen at Kind Clinic, including, but not limited to: preparing patients for examination, taking vital signs, verifying patient information by interviewing patient and recording medical history
  • Provide direct nursing care to patients, including the provision of lab services, and the distribution of medications and treatments according to accepted standards of care
  • Conduct patient follow-up visits and physical examinations according to accepted standards of care and within the definitions of the Texas Nursing Practice Act
  • Teach and counsel patients concerning health assessment and care including prevention of disease, disability, hygiene, and health maintenance in a safe, affirming, and culturally appropriate manner
  • Provide clinical training to new Medical Assistant staff and relevant feedback to the Clinic Administrator regarding training progress
  • Adequately reviews and prepares charts prior to daily visits
  • Serve as a mentor to clinic support staff
  • Act as a liaison and collaborate with Patient Support Center RN team as needed
  • Follow organizations clinical SDOs, policies and procedures when interpreting lab results, sending medication refills or scheduling treatments
  • Follow Texas DSHS notifiable condition verbal reporting requirements related to in-clinic diagnoses of presumptive primary syphilis and acute HIV
  • Prioritize work tasks and services and completes tasks in a timely manner
  • Maintain accurate, clear and concise progress notes, problem lists, and medication lists
  • Communicate appropriately and tactfully with staff, patients, partners, and community.
  • Actively engage in a quality improvement culture within department/organization and participate in identified quality improvement activities
  • Participate in staff, planning, in-service, and other meetings as needed
  • Perform other duties as assigned

Your previous work life and education must include:

  • Active RN nursing licensure that is valid in the state of Texas
  • 2+ years patient care experience in an outpatient clinic setting with marginalized populations

Your previous work life and education ideally include:

  • Bilingual English/Spanish
  • Experience in HIV testing, HIV services, STI testing, and treatment clinic
  • 2+ years of experience in working with diverse populations, including LGBTQ+ and other marginalized populations

Additionally, this position will provide a wide variety of collaborative support for Enterprise Clinical Operations to ensure applicable program requirements and operational processes are met. This position will proactively work across the organization to drive short- and long-term operational success of program initiatives. Primary Responsibilities: * Assist Enterprise Clinical Operations leadership and other related departments in the implementation of program initiatives and processes that support care delivery, patient safety, and quality of care in the clinic setting including planning, development, coordination, multi-disciplinary collaboration, monitoring and evaluation * Develop and support the dissemination and education of evidence-based tools, resources, policies, and procedures, standing delegation orders and standard operating procedures that impact care delivery and support clinical decision support systems * Support PCMH Manager to oversee nursing educational forums and strategies that ensure quality, safe nursing practice and utilization in the clinic setting * Collaborate with internal and/or external customers (e.g., business partners, clinical staff, physicians, Care Management, and other organizational leaders) to support Joint Commission standards and Patient Centered Medical Home (PCMH) care model principles and efforts * Act as subject matter expert for Joint Commission standards, PCMH principles, scope of practice, and regulatory requirements related to care delivery and practice * Participate in efforts to identify, propose, and evaluate targeted admission and readmission rates, health outcomes, patient satisfaction interventions, staff burnout and ambulatory care sensitive conditions, and identify and/or develop process tools and resources to support related care delivery * Evaluate and communicate identified opportunities to improve processes or tools to appropriate team members with solution options for consideration * Obtain input and feedback from stakeholders during Joint Commission, PCMH, Operations, Health and Safety, and other relevant meetings * Deliver formal presentations related to proposals / recommendations to build support for the PCMH initiative, reduce nurse role ambiguity and enhance care delivery and practice standards * Perform all other related duties as assigned This is an office-based position, Oaks III / Network BLVD., 78249 / Office park located at IH 10 East and De Zavala Road / 25% Local and Non-Local travel is required as determined by the business.
