Position Summary

The RN Supervisor is responsible for the coordination of the patient's care and services. Promotes quality care using a collaborative process that coordinates, monitors, and evaluates services according to the needs of patients. Works together with the patient and family in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

  • Associate's or bachelor's degree in nursing from an accredited college or university

  • Currently licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.

  • Two (2) years' nursing experience. Geriatric nursing experience preferred.Must have CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Specific Job Requirements

  • Expert knowledge in field of practice

  • Make independent decisions when circumstances warrant such action

  • Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility

  • Implement and interpret the programs, goals, objectives, policies, and procedures of the department

  • Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation

  • Maintains professional working relationships with all associates, vendors, etc.

  • Maintains confidentiality of all proprietary and/or confidential information

  • Understand and follow company policies including harassment and compliance procedures

  • Displays integrity and professionalism by adhering to Life Care's Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training

Essential Functions

  • Provide, manage, and coordinate patient care and services through interpersonal contact, which allows patients to attain or maintain the highest practicable physical, mental, and psychosocial well being

  • Assist patients daily to evaluate quality of patient care rendered

  • Direct licensed staff and CNAs in prioritizing patient care needs/reporting requirements

  • Assess patients' needs and develop plans of care

  • Exhibit excellent customer service and a positive attitude towards patients

  • Assist in the evacuation of patients

  • Demonstrate dependable, regular attendance

  • Concentrate and use reasoning skills and good judgment

  • Communicate and function productively on an interdisciplinary team

  • Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours

  • Read, write, speak, and understand the English language

VIEW JOBS 6/15/2022 12:00:00 AM 2022-09-13T00:00 What You Will Do - Friday through Sunday 8am - 8pm (3-12s) As a hospice registered nurse for Amedisys, you have the special opportunity to provide one on one hospice and palliative care to terminally ill patients in their home. In collaboration with a full hospice interdisciplinary team, your work will including the development and delivery of a patient specific care plan for the patient and caregivers. * Communicates significant findings, problems, changes in patient's condition or environment to the Clinical Manager/Physician and other members of the health care team. * Performs initial nursing assessment on new admissions and completes initial plan of care. Coordinates the total Plan of Care and maintains continuity of patient care by collaborating with appropriate staff. Revises plan of care as needed. Completes updated Plan of Care according to state regulation and communicates changes to attending physician, hospice staff, and contractors providing patient care. * Initiates appropriate palliative nursing procedures. Administers medications and treatments as written in the patient's plan of care. * Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so. * Provides education/teaching to families/caregivers/patients related to appropriate patient care techniques. * Utilizes a combination of care center resources and rationally recognizes standards of practice to achieve positive clinical outcomes. * Participates in nursing service evaluations, follow ups, care center performance improvement initiatives, and period quality assurance reviews. * Initiates, plans, and provides staff education and performs competency and skills validation for staff as needed. * Assists in assigning LPN and Hospice Assistants to individual patients and supervises their care. * Participates in care center QAPI activities and interdisciplinary team meetings. * Performs on-call responsibilities and on-call services to patients/families as assigned. * Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program. * Performs other duties as assigned. Required * Current, unencumbered license to practice as a Registered Nurse specific to the state(s) the employee is assigned to work/cover for the company. May possess either a single state license in each non-compact state and/or a compact license in their state of residence that is accepted in all Nurse License Compact (NLC) participating states. * Nurse License Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in an NLC state to be eligible to have a multi-state license. * One (1 ) year of experience as a Registered Nurse or eligible for RN intern program. * Current CPR certification. Preferred * Knowledge of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers. * Prior Hospice experience. * Computer literate, proficient in windows navigation, emails, internet and documenting in an EMR.
