Registered Nurse (RN) Supervisor Nights

Far Rockaway Center is seeking a Registered Nurse (RN) Supervisor to work for our Skilled Nursing Facility in Far Rockaway, NY.

Shifts Available:

3 PM - 11 PM
11 PM - 7 AM

Duties:

Handle all supervisory duties for the assigned unit.
Help establish and implement employee policies and procedures.
Mentor less experienced nurses, offering clinical & career advice.
Maintain the standards of care for the unit.
Review of Resident records & quality of care.
Monitor overall care & review individual Residents' cases
Address questions or complaints brought forward by Residents or their families.
Represent the unit's interests with upper-level management.

Requirements:

Must hold valid NYS RN License.
2 years Long Term Care Experience preferred.
1-year Charge Nurse experience preferred.
Should have an excellent positive attitude to act as the leader of a group.
Strong computer skills.
Excellent communication skills.

Role Description: Oak Street Health Nurse Practitioners are an integral member of our Care Teams, which also include Physicians, Registered Nurses, Medical Assistants, Clinical Informatics Specialists, Phlebotomists, and Care Managers. NPs see patients independently and alongside the doctors, depending upon the patient's specific situation. Nurse Practitioners provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patient understand and feel connected to their care plan. Together this team is responsible for providing and coordinating care for an intimate panel of patients in our neighborhoods. While the typical primary care panel in the U.S. is around 2,500 patients for a single physician, our panels at Oak Street Health are around 500-750 for a team. This creates an opportunity to spend more time with patients, build deeper relationships, and to better execute/coordinate care plans. Nurse Practitioners practice exclusively in our outpatient practices. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our NPs, with our doctors, focus exclusively on care within the clinic: primary care, care coordination, and population health.
