Job Summary

Job Summary

Provide for the medical care of residents so health is maintained at an optimum. Promote and encourage quality of life for each resident by following a social model of care.

Qualifications

Minimum Qualifications:

  • Licensed in Minnesota as a Registered Nurse, in good standing

  • Knowledge of nursing practices and techniques sufficient to provide effective nursing care for residents

  • Skill in establishing and maintaining positive working relationships

  • Effective communication skills sufficient to provide and follow instructions

  • Computer experience sufficient to complete electronic charting and use basic computer functions such as email

  • Ability to make decisions within the scope of nursing responsibilities

  • Ability to evaluate the effectiveness of nursing care provided by other staff, make or recommend changes, and provide mentoring

  • Ability to teach self-help skills to residents

Preferred Qualifications:

  • Experience in a skilled nursing facility

  • Long-term care experience

  • Geriatric care experience

  • Experience with MDS assessments

Addtional Qualifications:

  • A Minnesota Department of Human Services (DHS) background check will be conducted on finalists for this position. Finalists must pass the DHS background check.

  • Finalists will be required to pass a job-related pre-employment physical assessment.

  • A Tuberculosis (Mantoux) screening (or chest x-ray) is required prior to employment.

  • Employment reference checks will be conducted on finalists. This may include a review of documentation related to job performance and contact with the applicant's former employer(s).

Collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. Cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up as patients move along the continuum of care. Responsible for the planning and coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct. POSITION RESPONSIBILITIES Performs age related patient assessments and nursing interventions.;Offers effective feedback to patient and family members regarding health.;Establishes and maintains productive working relationships within and outside of own area.;Effectively delegates and supervises based on one's state practice act and an understanding of the concepts of responsibility, authority, and accountability.;Delivers written and oral communications that engage audience participants, respond to their questions and concerns, and produce specific outcomes and impact.;Coordinates and manages care for patients, including those with complex needs.;Adheres to legal regulations and organizational policies when discussing patient health information.;Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.; QUALIFICATIONS Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's Degree in nursing preferred. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
