Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Registered Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.

Our home health agency is a CARF accredited agency that is hiring a home health registered nurse that is seeking a patient-centered care model of rehab nursing. We work with patients of all ages to provide safe, quality care in a patient's home within Grand Rapids, MI and surrounding areas. Our Nurses provide one to one care for rehab patients in the community setting. Nurses use their clinical judgment and assessments to coordinate the care of the patient with an interdisciplinary team.

Responsibilities:

  • Assess and educate patients each visit/shift and document at point of care

  • Perform evaluation tasks, including vitals and med review

  • Administer and educate patient or caregiver on medications ordered by physician

  • Wound care

  • Infusion therapy

  • Respiratory care

Basic Qualifications:

Education

  • Associate's Degree or equivalent in Nursing

Licenses

  • Registered Nurse (RN)* State of Michigan

Drivers License MI (MI DR LIC)

Basic Life Support (BLS) Specific to VNA and Home Care, 90 Days

Preferred Qualifications:

Education

  • Bachelor's Degree in Nursing

Experience

  • 1 year of experience typically gained through skills/knowledge/abilities in of acute care, emergency, or rehab facility experience for all of the Home Care business lines

Home Health Aides work in assisting with task directly essential to the client's health in accordance with their care plan; observe, record, and report services provided and clients response to care as well as client status to the case manager.
