Case Management and Supervision

  • Follows VITAS policies and standards (See VITAS Standards Manual) and practices within the scope of the RN license

  • Reports any request to practice outside of scope of license to the manager/supervisor

  • Supervision of (VITAS employed and contracted) nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses (and contracted Registered Nurses) and Volunteers per the plan of care

  • Completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement

  • Appropriately identifies/utilizes resources, referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults

  • Completes documentation at the time of the event and at the location of care unless otherwise directed

  • Provides education to patients, families, caregivers and referral sources

  • Participates in patient care meetings as assigned

  • Participates in weekly team meetings

  • Provides effective communication/shift reports through written, verbal, reporting and body language

  • Participates in bereavement support

  • Participates in the on-call rotation and/or provides shifts during non-business hours such as evenings, weekends and holidays based upon program-identified needs

  • Provides visits/shifts in program coverage area

  • Develops, coordinates, and follows the patient's individualized plan of care

  • Appropriately delegates nursing tasks to Hospice Aides/Homemakers, Volunteers, and Licensed Practical/Vocational Nurses

  • All factors pertinent to the patient's safety, including the competence of the Hospice Aide/Homemaker have been assessed

  • Determination that this activity can be delegated safely to a Hospice Aide/Homemaker

  • Connecticut -For activities other than those listed in section 1913D69(d)(3)of the Connecticut Department of Public Health delegated to a Hospice/Homemaker, the RN will document in the clinical record that:

  • Provides nursing care including nursing tasks that may be delegated to other roles such as LPNs, HHAs and Volunteers

  • Participates in and monitors patient and family care compliance activities

  • Professionally manages/coordinates the hospice plan of care

  • Provides and follows infection control practices

  • Delivers care in accordance with the patient's plan of care

  • Provides primary nursing care for inpatient and continuous care patients as assigned

Quality Improvement

  • Participates in the Quality Assessment and Performance Improvement (QAPI) Program and related activities as assigned. This may include, but is not limited to
  • participation in Performance Improvement Project(s) (PIP)
  • Gathering clinical data which is used for quality measures

Professional Development

  • Develops and achieves professional growth goals and objectives personally and per Team Manager reviews

  • Responsible for attending orientation and regularly scheduled in-services/educational conferences as assigned

  • Participates in orientation of Hospice Aides, Volunteers and other IDG members as assigned

  • Participates in training activities internally within VITAS and externally as assigned

Customer Service

  • Maintains focus on the patient and family experience of care

  • Serves as a patient and family advocate

  • Reports patient/family/customer complaints to Team Manager

  • Participates in patient/family/customer conflict resolution as applicable.

  • Represents the nursing profession, the VITAS site/location and VITAS overall in a positive manner


  • Currently licensed as a R.N. in the state where the VITAS program is located.
  • A minimum of two years experience in hospice, home health, or community health experience in the last five years.

VIEW JOBS 9/15/2021 12:00:00 AM 2021-12-14T00:00 * Interview, hire, train, support, and develop all Customer Service Supervisors (CSS). * Manage all day-to-day CSS activities to ensure an efficient and timely intake/admission process is achieved with all new patient referrals. * Manage CSS adherence and compliance to all education, quality, workforce management, customer service, operational standards, and KPIs. * Continuously monitor all day-to-day referral activities taking place within the Care Connection Center, to include count of referrals received, status in real time, and next step, and follow up on previous days' and review those not admitted. Ensure complete to end process. * Lead by example to ensure a strong understanding and connection to the Vitas mission, core values, and Care Connection Center cultural platform and initiatives. * Supports all strategies, plans, and implementations as directed by Senior Leadership. * Emphasis on the supervision of VX data integrity to ensure a clean billable patient record. * Monitor and manage all inbound call volumes and pending referrals to ensure operational service levels are met/exceeded. * Ensure effective use of all development, counseling, and rewards and recognition tools to most effectively support all CSS team members. * Support the Care Connection Center orientation and training programs to ensure that knowledge, skill, and performance standards are met and/or exceeded. * Proactively identify operational efficiency and customer service enhancement opportunities. * Work directly with all corresponding programs Admission Managers to support, meet, and/or exceed their individual referral/admission goals. * Supports an enhanced partnership and bridge between the CCC Management team and the Program Management Team. * Responsible for identifying specific opportunities by Program and with specific referral sources during the day-to-day operations. * Respond to complaints in a timely manner and maintain quality improvement documentation. * Support all aspects of financial management, employment process, quality assurance and quality improvement, payroll, admissions, clinical staff, system operations, business plan development and implementation, human resource policy, and staff development/retention. QUALIFICATIONS * Three or more years related experience managing an operations team in a 24/7 environment. * Call center/customer service and/or healthcare management experience preferred. * Proven success in managing strategic plans and attaining goals in a fast-paced, dynamic environment. * Expert in the delivery and monitoring of customer interactions. * A solid track record of managing customer service complaints * Experienced in leading, managing, and supporting customer service supervisors. * Ability to research and analyze information and data to arrive at and articulate valid findings. This includes root cause analysis, to build recommended corrective action plans. * Ability to exercise considerable judgment and discretion in establishing and maintaining strong partnering relationships with team members. * Superior ability to effectively communicate at all levels both verbally and in writing * Ability to develop spreadsheets, and use word processing and database computer software as well as MS Excel, Word and PowerPoint. * Prior experience in working collaboratively with other functional leaders to drive strategic initiatives and action plans. * Proven track record for supporting teams that are proactive in driving process improvement. * Must be highly organized and able to manage multiple responsibilities. * Model leadership behaviors supporting: integrity, valuing diversity, innovation, building relationships, customer focus and teamwork. * Ability to work weekends and holidays as necessary to support the operations of the Care Connection Center.
