Currently seeking Telemetry (Tele) Registered Nurses (RN) for a contract position in Los Angeles, CA. This is a per-diem contract assignment with the 12-hr shifts. The ideal candidate will possess a current CA Nursing License and have at least 2 years of recent Telemetry (Tele) Registered Nurse (RN) US nursing experience as an RN.

RN must be available to work any of the following locations during the assignment as needed:

  • Oakland
  • Fremont
  • San Leandro
  • Walnut Creek
  • Richmond, Antioch
  • Flexibility and wiliness to float to any and/or all locations a must
  • You will received a schedule via email by Friday for the following week with assigned locations and dates
  • FYI you may be called up to two hours before your scheduled shift to be rerouted if needed
  • No time off will be approved for the duration of the assignment

Requirements of the Telemetry (Tele) Registered Nurse (RN):

  • Current Resume
  • CA Nursing License
  • A nursing degree from an accredited nursing program
  • Minimum 2 years of recent Telemetry (Med/Tele) nursing experience as an RN
  • Current BLS and/or ACLS and/or Specialty Certifications
  • Two current clinical references
  • Must be able to pass background/urine drug screening
  • Knowledge of state and federal regulations
  • This package is for 36hr - 10 week contract

Responsibilities of the Telemetry (Tele) Registered Nurse (RN):

  • Evaluate and oversee patients in the Telemetry (Tele) Registered Nurse (RN) and report to supervisors any changes in condition
  • Maintain and calibrate Telemetry (Tele) Registered Nurse (RN) equipment for patient safety and support
  • Administer medications as prescribed by attending Telemetry (Tele) Registered Nurse (RN), the physician
  • Connect intravenous lines to administer medication and extract samples
  • Communicate with parents/guardians the plan of care and scope of treatment
  • Obtain and evaluate test results
  • Consult with physicians and other nurses on a plan of care, progress, and prognosis

TITLE: Lead Hospice Triage Nurse (Remote) OFFICE LOCATION: Oakland PATIENT TERRITORY: San Francisco Bay Area, Remote SCHEDULE: Weekend & Holidays SHIFT: 8AM - 5PM - Every weekend and holiday POSITION SUMMARY: The Hospice Weekend/Holiday Triage Lead is responsible for the coordination of service delivery during regular business hours when administrative staff is not present, to Hospice patients, families, and facility staff. Provides clinical and administrative direction to field staff. Works from the office or home. ORGANIZATIONAL RELATIONSHIPS: * Receives direction and supervision from the assigned Afterhours Supervisors, Afterhours triage nurses. * Provides direction to Weekend Visit and Triage Nurses. AREAS OF RESPONSIBILITY: * Maintain current knowledge of regulations pertaining to the delivery of Hospice. * Accepts incoming calls from patients and/or their family members. Assesses information provided and recommends appropriate nursing interventions. * Consults with Interdisciplinary Group (IDG) members as indicated to determine appropriate interventions to be taken and makes recommendations. * Communicates with the Hospice medical director. * Coordinates with clinical providers to ensure pre-scheduled visits to patients are timely and based on patient/family need. * Accepts incoming phone calls from members of the community inquiring about hospice services. Provides appropriate information. * Accepts referrals from and provides information to physicians, discharge planners, and other sources for eligibility and appropriateness for hospice and communicates information to intake staff if needed. * Assigns unplanned visits for current patients in a timely manner based on the clinical needs of the patient, staff competency, staff workload, travel time, and the geographic location of the patient. * Accepts and documents physician verbal and written orders and obtains other patient care information from physician/patient/family. Communicates new orders/information to the case manager or appropriate IDG member, including pharmacy providers. * Provides clinical consultation as needed for clinical providers and office staff. * Knowledgeably discusses overall history, philosophy and services of Pathways Home Health and Hospice. Supports, instructs and educates the patient, family, caregivers, and facility staff. * Documents nursing assessments, problems, appropriate goals, care interventions, patient and family outcomes, and evaluations of these in the electronic record. * Accesses electronic device during patient communication/interaction and documents during or immediately after the communication/ interaction or per regulation. * Maintains the dignity of the dying patient and supports the patient's and family's unique spiritual and cultural beliefs. * Supports holistic, family-centered care across treatment settings to improve the quality of life. * Knowledgeably discusses and initiates non-pharmacologic interventions as appropriate to promote comfort, improve quality of life, and help manage emotional, spiritual, and physical distress. * Participates in the hospice program's quality assessment performance improvement program and in-service education related to areas of practice as appropriate. * Meets productivity standards as established by the Agency. * Maintains timely communication with the patient, family, and facility staff regarding all aspects of patient care. * Creates, documents, and maintains the medication list. Evaluates medications for side effects or interactions as able and communicates these. Effectively instruct the patient, family, and/or staff use of medications and reconciles medication records. * Effectively communicates with and uses the hospice prescription plan, formulary/preferred drug list, and general hospice medication coverage philosophy. QUALIFICATIONS: * California RN license required: PHN preferred. * Recent experience in home care or hospice preferred. * Minimum of two years of nursing experience. * Supervisory experience preferred. * California driver's license and automobile insurance with minimum Bodily Injury Liability Insurance per agency policy. * Current CPR certification – American Heart Association "BLS Health Care Provider" required upon renewal.
