Job Summary

Function as clinical leader and liaison with staff, physicians and management, providing continuity of leadership day to day. Act as a role model, mentor, coach and resource person to staff to ensure all unit standards are maintained and to encourage team environment. Assist manager in attracting, developing, and retaining qualified staff members. Duties may include assisting with hiring, orientation, monitoring, and evaluating performance, problem solving, employee counseling and staffing. Responsible for maintaining a safe environment that fosters positive customer service and meets all quality standards, implementing new and monitoring ongoing processes.

This role provides opportunity for leadership succession.

  • Account for and coordinates day-to-day operations including patient flow, staff assignments, projecting staffing needs, adjusting staffing for volumes. Is "in charge". Delegate and supervise care. Coordinate operations with other area(s) to ensure continuity of care for the patient.

  • Serve as a clinical expert, mentoring associates, maintain positive relationships and build teamwork. Acts as a role model, coach and evaluates staff performance. Provide ongoing feedback to staff related to job performance and departmental processes (focuses on real-time performance coaching, administer focus feedbacks if necessary)

  • Maintain knowledge of regulatory standards, maintain department in survey readiness, policies, procedures, and standards. Evaluate quality and appropriateness of care. Conduct audits for regulatory compliance as well as department quality of care.

  • Coordinate with staff educator to meet educational needs in new policies, procedures and techniques. Provide "just in time education". Participate in associate interviewing, orientation, and provides input for performance reviews.

  • Evaluate/ Function as a preceptor in orienting new associates and assisting with the completion of the orientation checklist. Assist with Return Demonstrations/Clinical Observations and training.

  • Co-facilitate planning and conducting of shared governance meetings. Create and supports and environment where innovation and creativity are encouraged and valued. Participates in clinical care committees.

  • Assist with ensuring that assigned area(s) are supplied with all the equipment and materials necessary to provide the level of nursing care required in the care plans.

  • Participate in physician rounding and in leads in Multi-disciplinary patient rounding. Confer with physicians and other personnel concerning patient care or other matters relating to assigned area(s). Provide professional nursing care as required and can fill in as staff nurse when needed.

  • Understand patient satisfaction scores; and can relay this in practical terms to the staff.

  • Effectively communicates through difficult situations and effectively manages conflict.

Job Details Location: Stevensville, MI Department: LTC Nursing Administration - Director Nursing Pine Ridge Status: Part time Shift: Day (United States of America) Hours: 6 am to 2:30 pm Job Summary The LPN may work under the direction of a charge nurse or may be a charge nurse. Provides a broad variety of direct and indirect care. This may include delegated components of the assessment and discharge procedures, aide supervision, basic patient education, unit specific treatments and diagnostic modalities, activities of daily living, and dietary assistance. When functioning in the capacity of a charge nurse is responsible for the smooth functioning and supervision of the entire unit. * Ensure all new Admissions to Pine Ridge have the RAI (Resident Assessment Instrument, MDS and CAAs) completed in accordance with the MDS process. Ensure all Residents have RAI's completed accurately within the appropriate time frame per protocol. This may involve working during non-business hours to meet billing deadlines. • In conjunction with the floor/admitting nurse, ensures that the 24 hour initial care plans for Residents are completed according to the regulatory time table and Resident needs. • Verify that the MDS has been opened within 24 hours of admission. • Serves as a resource person to staff to assist in providing accurate clinical direction and care plan implementation. Provides direction and counsel to members of the care team when appropriate. Documents corrective counsel provided and provides copy to D.O.N. • Attends Case Review weekly meetings and assures the team is completing the MDS by the assigned time per MDS protocol. • Assess Resident care delivery in order to evaluate the development and implementation of care plans, provides feedback to staff on methods of care and Resident education, and ensures that a safe environment is maintained for staff, Residents and family • Will attend care conference meetings if needed, and be a strong Resident advocate. Be able to discuss all relevant care issues including changes in medications, treatments, etc. Document on care conference notes all relevant nursing questions and concerns, and provide assurance concerns will be addressed. Follow up with ADON and/or Charge Nurse. • Complete all Resident Care Plans in a timely manner. Care plans should be person centered and highly individualized to reflect an accurate picture of resident problems along with realistic goals and interventions that reflect the resident's choices and abilities. Assure that all relevant triggered Quality Measures, CAAs as well as medical conditions, special needs, etc. are all thoroughly care planned and interventions to manage identified care needs are put in place. Work with the Charge Nurse/ADON on any new changes in Resident condition that need to be addressed. Ensure that the resident and/or their advocate is aware of care needs and involved in the care planning process. Assist all residents to achieve their highest practicable level of function. • At quarterly and annual care conferences, ensure all information on the face sheet is accurate and update the Aide Care Instructions/Profile/Kardex at that time. Ensure that names, phone numbers, advanced directives and medical diagnoses are up to date. Review all medications, especially psychotropic, documenting the risk/benefit discussion with Resident and family. • Work closely with the Wound Nurse to share accurate information on pressure ulcers. Work with Wound Care Nurse to make sure care plans are updated and reflect current care. • Review all past goals prior to quarterly care conferences and update goals as necessary. Discuss any changes or possible new problems with the ADON and Charge Nurse. • Attend weekly Rehab Rounds meetings to ensure proper usage of Medicare Part B benefits in accordance with State and Federal statute. Ensure Residents' Rehab needs are being met with quarterly screening for functional decline/improvement. Ensure that Medicare Part B usage does not exceed the cap if the diagnosis is not Yes. Utilize restorative and/or functional maintenance programs as appropriate.
