Presbyterian Homes & Services is seeking a Registered Nurse to join our team at Carondelet Village in St. Paul, MN. Currently hiring for a RN with 1+ year of nursing experience for:

  • A Part Time, .6, Overnight in LTC (10:45PM-7:15AM).

Responsibilities:

The Registered Nurse (RN) is responsible for the provision and monitoring of high quality clinical care and services to residents consistent with regulations and established clinical best practices.

Qualifications:

  • Registered Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.

  • Current CPR certification.

  • Interest in gerontological nursing, rehabilitative and restorative clinical practices.

  • Demonstrated compatibility with PHS's mission and operating philosophies.

  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

VIEW JOBS 8/1/2021 12:00:00 AM 2021-10-30T00:00 Our established home care community in Inver Grove Heights, MN is accepting applications for a Full - Time RN Case Manager to join our team and oversee the delivery of quality health care and related services within the Inver Grove Heights-area and surrounding communities. SCHEDULE: Hours are primarily Monday-Friday, day-shift schedule but may require additional hours based on client needs. This position requires an on-call rotation. POSITION SUMMARY: The Case Manager is responsible for ensuring that the health and safety of all clients are met through assessments and reassessments of the clients, development and implementation of appropriate service plans, monitoring of clients' services, and the supervision of home care staff. This will assist each client to reach and maintain his or her highest level of performance and independence. RESPONSIBILITIES: * Completes initial, face to face comprehensive assessment of prospective clients in accordance with agency policy. Reassess clients after initiation of services and as needed. * Develops, reviews and implements service plans with all assigned clients. * Assures clients have up-to-date care plans that address their needs and preferences. * Communicates with physicians, families and other personnel regarding changes in client's needs or conditions. * Assures that each home care staff person is oriented to the individual needs of each client. * Completes all required electronic charting and paperwork in a timely and legible manner. * Perform other functions as deemed appropriate by the Management or Executive Team.
