Registered Nurse (Rn) - Long Term Care

Job Overview: We are seeking a compassionate and dedicated Long-Term Care Registered Nurse (LTC RN) to join our healthcare team. The LTC RN is responsible for providing high-quality care to residents in a long-term care facility, ensuring their physical, emotional, and social needs are met.

This position involves coordinating with interdisciplinary teams, monitoring patient health, administering medications, and assisting residents with daily activities to maintain optimal quality of life. Key Responsibilities: Deliver comprehensive nursing care to residents in accordance with their care plans and nursing standards. Monitor and assess residents health status, identifying changes in condition, and promptly reporting concerns to physicians and care teams.

Administer medications and treatments as prescribed, adhering to established protocols. Assist residents with daily living activities such as bathing, dressing, eating, and mobility. Develop and maintain strong relationships with residents, families, and healthcare professionals to promote a supportive care environment.

Coordinate and participate in care conferences with residents, families, and healthcare teams to ensure holistic care planning. Document care provided in electronic medical records (EMR) accurately and timely. Provide supervision and direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other care staff.

Ensure compliance with federal, state, and facility policies and regulations, including infection control and safety standards. Educate residents and families on care management, chronic illness, and health maintenance. Participate in ongoing training, staff meetings, and continuing education to maintain nursing licensure and stay updated on best practices.

Qualifications: Current and valid Registered Nurse (RN) license in the state of Delaware or compact license. 1-2 years of nursing experience, preferably in long-term care, geriatrics, or a related field. Basic Life Support (BLS) certification. Strong communication and interpersonal skills with a commitment to compassionate care.

Ability to work collaboratively in a multidisciplinary team environment. Excellent organizational and critical-thinking skills. Proficiency in electronic health records (EHR) systems and basic computer skills.
