Job Description:

The RN provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. The tasks and responsibilities include:

  • Assesses plans and evaluates patient care needs.

  • Carries out physician orders.

  • Administers prescribed medications, changes dressings, cleans wounds, monitors vital signs.

  • Serves as the primary coordinator of all disciplines for well coordinated patient care.

  • Monitors, records and communicates patient condition as appropriate utilizing computerized documentation systems.

  • Instructs and educates patients and families.

  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

  • Provides age and culturally appropriate care.

  • Orients and mentors new staff members.

  • Follows Standard Precautions using personal protective equipment as required.

Required Job Qualifications include:

  • Current licensure as a FL RN

  • Current certification as a BLS, ACLS, PALS Healthcare Provider.

  • Graduate of an accredited school of professional nursing preferred

  • Previous Imaging, Interventional Radiology, Neuro Intervention experience

  • Will consider training strong ICU RNs

  • Customer service abilities including effective listening skills.

  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment.

  • Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 pounds.

Benefits: We offer a generous compensation Job Description: The Care Assure Nurse Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. S/he facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The incumbent guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The Care Assure Nurse Navigator serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services. Responsibilities: * Reviews patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention, while tracking outcomes of referrals and treatments and maintaining detailed records that measure effectiveness of the program. * Reviews clinical progress of patients and coordinates communications to referring, consulting, and primary care physicians, notifying an MD if there is a clinically abnormal finding. * Evaluates and provides input regarding the utilization of resources to meet the needs of the target population. * Plan and delivers weekly status updates. * Maintains and role models a leadership/customer service role in interacting with colleagues, physicians, guests, patients, and vendors. * Bachelor's degree in Nursing preferred * Current Florida RN license required * 3 to 5 years relevant specialty area experience Cardiac/Cardiovascular preferred. * Knowledge of MS Office, scheduling systems, and hospital based computing systems (i.e. Meditech). * Critical thinking, service excellence and good interpersonal communications skills. * Ability to read/comprehend written/verbal instructions. * Demonstrates advanced computer skills. * Leadership skills to direct others and maintain a common vision for project objectives.
