Registered Nurse (RN) House Mentor

The award-winning Leonard Florence Center for Living Green House is a revolutionary, state-of-the-art skilled nursing residence. As the first urban Green House skilled nursing facility in the country, the Center is dramatically different from a traditional nursing home.

Leonard Florence Center for Living in Chelsea, seeks a House Mentor to join its team. The right person will have a strong clinical background and supervisory experience.

Position Summary:

The House Mentor is responsible for overseeing the day-to-day department activities of a Green House Household. You will be working on a busy and vibrant short-term rehabilitation unit in a modern facility with staffing ratios above industry averages, an on-site medical group, and a unique "household" model of care. You will be part of a management team of proven, dedicated professionals committed to quality care and personal growth.


  • Assumes responsibility for all nursing care provided to residents within an assigned unit; knows the residents' conditions at all times; observes and supervises nursing care according to physician's orders and in conformance with recognized nursing techniques and procedures, established standards and administrative policies.

  • Recognizes and interprets symptoms, reports on residents' conditions and assists with or institutes remedial measures of adverse developments.

  • Assists in maintaining a physical, social and psychological environment which is conducive to the best interest and welfare of residents.

  • Investigates and studies trends and developments in nursing practices and techniques and assesses their adaptability to the needs of specific nursing policies.

  • Instructs staff in the use of policies and procedures and assures that all policies and procedures are followed by nursing staff.


  • RN graduate from an accredited school of nursing

  • Current RN licensed is required

  • One (1) to Three (3) years' experience as a supervisor, preferred

  • Must have excellent clinical assessment skills

  • Must be able to communicate effectively

  • Must be able to demonstrate organizational skills.


  • This is a full-time position, Monday-Friday with on-call rotating weekends

The Leonard Florence Center for Living sets the stage for a model of care far removed from traditional nursing homes. It is not only the first urban Green House skilled nursing facility in the country, but it also has the remarkable distinction of showcasing three homes dedicated to caring for individuals living with amyotrophic lateral sclerosis (known as ALS or Lou Gehrig's disease) and multiple sclerosis (M.S.). Residents who are completely immobilized are able to control the lights, turn on the TV, open doors and raise window shades just by using their eyes. In addition, there are three homes dedicated to short-term rehabilitation and four homes dedicated to long-term care for seniors. Named after the late Chelsea-born philanthropist Leonard Florence, The prevailing goal of the Leonard Florence Center was to create a "real" home that provides exceptional care, but also supports those seeking to redefine the worth and meaning of late life and those living with debilitating diseases. Executive Director Position Summary: The Executive Director is responsible for directing the day-to-day functions of a state-of-the-art, six-story, 100-bed Green House skilled nursing facility that embodies the many comforts, design and ambiance of one's personal home. As the Executive Director, you will make decisions that make a difference to our employees, residents and families. The function of the Executive Director is to ensure delivery of the best quality care for our residents. This position will develop, direct, and control facility operations within the bounds of established policies, procedures, programs, and budgets. Responsibilities include but are not limited to: * Ensure regulatory compliance with annual surveys * Represent the Organization both onsite and in the community * Provide systems for continuing assessment of residents' health and well-being * Maintain regulatory compliance in accordance with state and federal guidelines * Assure a safe and pleasant environment for residents, family members and employees * Conduct routine inspections of the facility to ensure that all policies and procedures are being followed * Assume responsibility/authority for decisions regarding all aspects of facility operations * Manage budget to meet facility needs and goals, working with CFO and department managers * Develop and maintain positive relationships with residents, family members, employees and medical team * Conduct routine inspections of the facility to assure that established policies and procedures are maintained * Work in partnership with the Director of Nursing and Chief Operating Officer to ensure quality, person-centered care * Establish and direct the implementation of written policies and procedures that reflect the goals and objectives of the Organization Qualifications: * Bachelor's Degree * Demonstrated impeccable leadership and organizational skills * Current Nursing Home Administrator license required * Three (3) to five (5) years of experience in an Executive Director role * Effective communication and interpersonal skills are a must * Demonstrated ability to lead and supervise professional and administrative staff Chelsea Jewish Lifecare is an equal opportunity employer.
