Ecumen Hospice is currently seeking a part time, hospice visit nurse to join its growing team providing services in Southern Minnesota!

Ecumen Hospice is a non-profit, Medicare-certified hospice that has been providing life-honoring care in Minnesota for almost 30 years. We serve those who are experiencing a life-limiting condition and the people that are closest to them, through a comprehensive offering of services which include but are not limited to: physician oversight, nursing, social work, spiritual care, hospice aide assistance, music therapy, massage therapy, volunteer services, pet therapy, and bereavement care.

Responsibilities

The Registered Nurse (RN)-Hospice works from home and is responsible for the delivery of patient care including; triaging patient needs over the phone, making visits to patients with changing conditions, and performing admissions as necessary.

Essential job responsibilities include:

Receives and responds to phone calls during assigned shifts as well as making visits to patients with changing conditions, and completing weekend admissions. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness. Assesses and evaluates patient's status by:

  • Writing and initiating plan of care.

  • Regularly re-evaluating patient and family/caregiver needs.

  • Participating in revising the plan of care as necessary.

Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Initiates the plan of care and makes necessary revisions as patient status and needs change. Provides health care instructions to the patient as appropriate per assessment and plan.

Qualifications

Registered Nurse (RN)-Hospice

Minimum Required Qualifications:

  • Graduate from an accredited Registered Nursing program

  • Must be licensed as a Registered Nurse within the state in which working and must maintain licensure

  • One to two years of nursing experience in post-acute care

  • Current valid driver's license and proof of vehicle insurance.

  • Ability to communicate effectively in both verbal and written formats

  • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

Preferred Qualifications:

  • Bachelor's degree

  • One to two years of recent nursing experience in Medicare Hospice

  • Proven leadership abilities

Position description The registered nurse in the Outpatient Dialysis areas will have a home site of Owatonna; however will float to the other dialysis satellite locations. The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability, and adequacy of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects. A subset of employees may be required to drive their personal vehicle as a part of the responsibility of their role. Qualifications Student or graduate of an accredited, or those in the candidacy process, baccalaureate, associate or diploma nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Exceptions to this may be made in the MCHS sites prior to a law change requiring accreditation expected in 2018. If graduated from a nursing program that was not accredited by ACEN, CCNE, or NLN CNEA, at least one year of RN experience (within the last 5 years) in an applicable care setting is required. If graduation did not occur within the last two years, one year of RN experience in an applicable care setting is required, or one year of current LPN experience at Mayo Clinic is required. All associate degree or diploma registered nurses hired into the Department of Nursing with an RN start date after April 1, 2020, must provide documented evidence of program completion of their baccalaureate degree in nursing from an accredited nursing program within 5 years from the last day of the month of their RN start date. Additional qualifications Excellent communication skills (verbal and written.) Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to adapt to unpredictable situations within the work setting. Ability to work flexible hours. License or certification Must obtain prior to start date: Current RN license by applicable state requirements.
