Registered Nurse (RN) Responsibilities:

  • Follow a designated care plan in accordance with patient's needs

  • Makehome visits to clients in designated geographic territories

  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures

  • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager

  • Follow up with, execute, and properly document doctors' orders

  • Perform client assessments as necessary

  • Case management and coordination

  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

  • A current license as a Registered Nurse in Florida.

  • Prior home care experience strongly preferred, but not required.

  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.

  • Ability to work independently and manage time effectively.

  • Strong interpersonal skills.

  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.

  • Ability to travel to cases as assigned.

Incumbents collect and analyze data to identify individual and/or community needs prior to planning, implementing, monitoring, and evaluating programs and/or services designed to encourage healthy lifestyles, policies and environments. Incumbents may also serve as a resource to assist individuals, other professionals, or the community, and may administer fiscal resources for health or nutrition education programs. This position may also be responsible for coordinating work.</p><p>This position is part of a grant funded program called OD2A (Overdose Data to Action) and has current funding through August 2023. This funding contingent/renewable yearly based on grant guidelines.</p><p>This position plans and provides educational opportunities related to substance use and abuse. Collaborates with health specialists and civic groups, related to substance use and abuse, to ascertain community health needs and determine availability of services then develop goals. Promotes health discussions in schools, industry, and community agencies related to substance use and abuse. Conducts community surveys to ascertain health needs, related to substance use and abuse then determine availability of resources. Prepares and disseminates educational and informational materials on related to substance use and abuse. Participated in OD2A (Overdose Data to Action) program calls, trainings, and seminars. Develop partnerships within the community to assure the best use of OD2A promotion resources.
