Aevitas Home Health - Sign on bonus This is a unique opportunity We are seeking to hire an experienced Registered Nurse to join our ever-growing interdisciplinary team of Home Health and Hospice professionals. RN Case Management Expectations: Provides direct patient care in their home. Oversees the patients plan of care.

Coordinates the patients ongoing skilled care under the direction of patients' physician. Promotes positive patient outcomes. Participates in patient interdisciplinary case conference.
