Requirements: Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting. Current licensure in the State of Arizona.

BLS hands-on CPR certification. Valid driver's license and auto insurance. Associate degree with one year of home health care experience preferred.

Responsibilities: Initiate and revise the plan of care based on patient status and needs.

Develop a comprehensive care plan with patient and family involvement, focusing on palliative nursing actions. Provide counseling to patients and families to meet nursing and related needs.

Deliver health care instructions tailored to each patient's assessment and plan.
