Registered Nurse (Rn) - Home Health & Hospice PRN

Sierra Health Care, Inc. is currently seeking a compassionate and experienced Registered Nurse (RN) for our Home Health & Hospice program. As a Registered Nurse, you will have the opportunity to provide high-quality care to our patients in their home environment. You will be responsible for assessing patients' health needs, developing care plans, coordinating with healthcare providers, and providing ongoing support and education to patients and their families.

Requirements

Qualifications & Requirements:

• Current and valid Registered Nurse (RN) license
• Minimum of 1 years of nursing experience, preferably in home health or hospice
• Experience in case management and care coordination
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Valid driver's license and reliable transportation
• Compassionate and empathetic approach towards patients and their families
