POSITION SUMMARY

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes.

DUTIES AND RESPONSIBILITIES

  • Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).

  • Provides professional nursing care by utilizing all elements of nursing process.

  • Initiates the plan of care and makes necessary revisions as patient status and needs change.

  • Uses health assessment data to determine nursing diagnosis.

  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.

  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.

  • Counsels the patient and family in meeting nursing and related needs.

  • Provides health care instructions to the patient as appropriate per assessment and plan.

  • Assists the patient with the activities of daily living and facilitates the patient's efforts towards self-sufficiency and optional comfort care.

  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

POSITION QUALIFICATIONS

Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. Current licensure in state and CPR certification. Bachelor's degree, with one (1) year of home health care experience preferred. Management experience not required. Responsible for supervising home health aides. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Job Summary Under the direction of the Director of Rehabilitation and Director of Home Health. The Home Health Physical Therapist plans, organizes, develops, administers, directs, and supervises Physical Therapy services in accordance with current applicable federal, state, and local standards, regulations, and guidelines to ensure the highest degree of quality care is rendered at all times. Effective supervisory skills are needed, along with good communication skills. The individual in this position must have initiative, be innovative, possess a sense of responsibility, integrity and sound judgment; follow standards of proper conduct, and abide by facility policies and standards of care. This position does provide direct patient care to adult and geriatric patients, including supervision of treatment delivered by Physical Therapy Assistants. Qualifications Possesses a degree from a Physical Therapy program. Must have a California License to practice as a Physical Therapist . At least two (2) years of appropriate experience as a Physical Therapist is preferred. Possesses and maintains current BLS certification. Must be a California licensed driver with an automobile that is insured in accordance with California requirements and is in good working order. Demonstrates good verbal and written communication, and organization skills. Duties and Responsibilities * Provides Physical Therapy services to patients according to a written individualized plan of care. This may include, but not be limited to: * Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment. * Assesses for muscle strength, mobility, gait, ROM and transfer capabilities. Less a goal of rehab, more a focus on maintaining comfort. * Directing physical therapy treatment. * Provides equipment required to increase patient function and independence. * Instructing patients and families/caregivers in the use and care of therapeutic appliances. * Determining priority needs for physical therapy * Reporting to physician patient's reaction to treatment or changes in condition. * Initiates Physical Therapy program and instructs other personnel and/or family/ caregivers in certain phases of Physical Therapy with which they may work with a patient, as well as instructing them as to the goals of the Physical Therapy program for the patient by participating in case conferences. * May educate patient in the use of prosthetic device. * Prepares and submits clinical and progress summaries based on the attainment of goals. * Participates in patient discharge planning.
