  • High School Graduate or GED

  • Graduate of an approved school of professional Nursing

  • Current nursing license in good standing with the Board in the state of practice

  • Minimum of one (1) year of experience as a professional nurse

  • Proficient in knowledge and use of computers and EMR software

  • Ability to work independently with minimal supervision

  • Exceptional communication skills with focus on quality patient care and customer satisfaction

  • Is flexible, cooperative, team player in fulfilling all obligations


  • Conducts the admission of the client to the Agency by:

  • Obtaining a medical history, particularly as it relates to the present condition, from the client and/or family member(s)

  • Conducting a physical examination of the client, including vital signs, physical assessment, mental status, appetite and type of diet, etc.

  • Evaluating the client, family member(s) and home situation to determine what health teaching will be required

  • Evaluating the client's environment to determine what assistance will be available from family members in caring for the client.

  • Evaluating the client's condition and home situation to determine if the services of a home health aide will be required and the frequency of this service

  • Interpreting nursing and other services of the Agency to clients and families as a part of planning for care

  • Developing and implementing the nursing care plan

  • Evaluate clients for need relating to other disciplines such as PT/OT/ST/HHNMSW

  • Provides skilled nursing care as outlined in the nursing care plan to include the following:

  • Nursing services, treatments and preventative procedures requiring substantial specialized skill and ordered by the physician

  • Initiating preventative and rehabilitative nursing procedures as appropriate for the client's care and safety

  • Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs, and changes in the client's physical or emotional condition

  • Teaching, supervising and counseling the client and caregivers regarding the nursing care needs and other related problems of the client at home

  • Is responsible for the care given by the home health aide to clients in his/her care by:

  • Supervising and evaluating the care given by the home health aide as needed and at a minimum of once every fourteen (14) days

  • Submitting to the aid department written evaluations of the home health aides who are providing services to the clients in his/her geographical area

  • Participating in periodic conferences with the home health aide supervisor concerning the home health aide's performance

  • Charts those services the staff nurse has rendered to the client, changes notes in the client's conditional and/or family and home situation, makes revisions in the nursing care plan as needed, records supervisory visits conducted with the home health aide, evaluates client care and progress and closes charts of discharged clients

  • Prepares and presents client's records at the Case Management Conference as indicated

  • Evaluates the effectiveness of his/her nursing service to the individual and family

  • Consults with the attending physician concerning alteration of the plan of treatment in consultation with the supervisor

  • Submits clinical notes per the standard operating procedure and progress notes and other clinical records forms outlining the services rendered as indicated

  • Participates in case conferences

  • Discusses with the supervisor any issues or concerns relating to the clients and how they may best be handled

  • Discusses with the supervisor the need for involvement of other members of the health team such as the home health aide, physical therapist, speech therapist, occupational therapist, social worker, etc.

  • Obtains orders for paramedical service and submits referral to appropriate personnel

  • Coordinates total client care

  • Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan

  • Participates in staff development meetings

  • Continually strives to improve his/her nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual research and reading

  • Participates in the planning, operation and evaluation of the nursing service

  • Participates in the development and periodic revision of the physician's Plan of Treatment and processes change orders as needed

  • Maintain an on-going knowledge of current drug therapy

  • Prepare the care plan for the Home Health Aide

This position performs duties as assigned by the Charge Nurse, including assisting and supervising Patient Care Technicians. Acute programs may report directly to an Administrator or the first line of regional operational management which may be an Area Director, Regional Director, or Vice President of Operations, depending on the region/demographics. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and tasks may be assigned. GROWTH * Assist with clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments. * Demonstrate effective use of supplies and staff labor hours. * Perform duties as assigned to meet the patient care or operational needs of the clinic. OUTCOMES * Complete and document monthly review of patient medication profiles. Administer medications as ordered by the physician. * May assist physician during patient rounds and transcribe and implement physician orders timely. * Assist with the implementation of anemia management and medication protocols as requested. * Provide patient education and follow up as needed. * Assist with program's target goals for patient outcomes in accordance with quality patient care and Company goals. OPERATIONAL READINESS * Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements. * Perform duties at all times within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations. * Complete patient admission process as outlined in policy and procedure, collaborating with team members regarding real or potential problems or needs relative to the treatment provided. * Complete pre and post treatment physical assessments. * Initiate and terminate patient treatments to include connecting and disconnecting catheters and cleaning catheter sites per protocol. * Set up dialysis machines; report machine problems to biomedical technician and/or clinic's Administrator as appropriate. * Clean, disinfect and provide quality control of dialysis machines as required by policy and procedures. * Participate in infection control monitoring, implementation, and recording as requested. * Use personal protective equipment as necessary. * Be familiar with emergency equipment and all emergency operational procedures. * Demonstrates effective staffing, as appropriate with staffing ratios per state regulations and effective patient care, under the direction of the Administrator. PARTNERSHIPS * Communicate results of patient assessment, reassessment, and ongoing monitoring to the physician, team members, and others as appropriate to the individual needs of the patient. * Maintain collaborative working relationship with Medical Director and physicians. * Participate as a member of the Interdisciplinary team in all required patient assessment and care planning activities. * Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization. STAFF DEVELOPMENT/ RETENTION * Participate in staff meetings as required. Attend in-service and continuing education offerings in compliance with company policy and procedure. * May delegate tasks to competent licensed and unlicensed staff per applicable state practice act. * Assist with staff training as requested. * Lead staff in team concepts and promote a team effort. QUALIFICATIONS/REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Requirements include: * Current RN license in applicable state. License must be maintained as current and in good standing. * 12 months or more current nursing experience preferred. * Previous experience in providing nursing care to patients on maintenance dialysis preferred. * CPR certification required within 90 days of hire. * Confirmation of ability to distinguish all primary colors. * Demonstrated working knowledge of the English language and ability to communicate verbally and in writing. * Must have basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire. * Must meet any practice requirement(s) for the applicable state. * Ability to meet minimum hospital/agency hiring standards which may include additional background clearances and orientation requirements if applicable to the program assigned to for employment.
