Position Summary:

In this role, you will assume responsibility to lead our team in providing quality patient care. You will assess, plan, implement and evaluate patient care from admission to discharge and you will be responsible to direct, assign and supervise ancillary staff team members in providing the appropriate level of care in the most efficient manner. You will also act as a role model to staff members.

Scheduled Hours
36 hours per week, night shift. Every 4th weekend required and rotating holiday schedule

Qualifications

Qualified candidates will be a graduate of an accredited school of nursing

Current Massachusetts Registered Nurse license.

BSN preferred, BCLS & ACLS required.

Candidates must have 2-3 years of acute Med/Surg experience or at least 1+ years of critical care experience.

Position Summary: Case managers serve as the care coordinators for patients, caregivers, and families. Their functions include concurrent and retrospective review to assure appropriate utilization, assessment, prevention of denials and coordination of discharge options. They are required to follow specific criteria sets to assure regulatory compliance. They coordinate safe discharge plans between the health care delivery team, patients, families, insurers and continuing care agencies. Qualifications Education/Experience: 1. Registered Nurse - Bachelors degree and CCM preferred 2. Excellent clinical skills in acute hospital setting 3. Excellent communication/interpersonal skills 4.
