As a Home Health Registered Nurse, heres a big-picture view of what youll do: Provide home-based care to patients with illnesses, injuries and chronic diseasesoften after discharge from a hospital, rehab or nursing facility Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care Examine patients, do assessments, document progress and report changes to their physician Educate patients on their plan of care, prescribed medication, therapy, diet and exercise Coach family members on the patients plan of care, medication and home safety Ensure goals are met and coordinate discharge from services A few must-haves for Home Health Registered Nurses: Active Registered Nurse (RN)License in Colorado Home health care experience helpful but not required No Case Manager responsibilities Graduate of an accredited school of nursing At least 1200 hours of experience as an RN Health care CPR/BLS Certified(demonstration course required) Pass a background check and drug test (we are required to test for THC) No Covid Vaccination Required Must be willing to travel to Greely, Front Collins, Loveland and surrounding area Why Work for Interim HealthCare?
