Seeking a RN Case Manager to join our team who will work with patient/families identified to have discharge planning, psychosocial, financial or other needs to develop a plan that will: 1) Optimize functional and self-care abilities; 2) facilitate system access and appropriate utilization of services; 3) facilitate communication and coordination of efforts among health care team members; and 4) promote quality, cost effective outcomes. The Case Manager will demonstrate knowledge and skills necessary to develop a plan based on physical, psychosocial, educational and age related criteria.

What you will do in this role:

  • Assesses by interview and charts review discharge needs of all high-risk patients (and others referred). Identifies resources available to address the needs identified, coordinate referrals and document plan.

  • Utilize community resources to support patient's post hospital needs. Documents choice in available resources has been offered.

  • Performs admission and continued stay reviews. Provides clinical information to payers upon request for certification and reimbursement of care.

    Discusses alternate level of care with physician. Refers cases not meeting acute care criteria to Physician Advisor as needed.

  • Facilitates and or provides education of hospital staff, physicians, patients and caregivers for purposes of efficient resource utilization

  • Keeps current of requirements by JCAHO, HCFA and other state/federal agencies on provision of 2 care, planning and documenting.

  • Tracks resource statistics to identify improvement opportunities. Attends medical staff and other hospital committee meetings to report findings.

Qualifications:

Education:

  • Graduate of an accredited diploma, associates, or baccalaureate degree nursing program.

Experience:

  • Minimum of 3 years clinical experience in area in which Case Management will be performed.

License and Certification:

  • Current Texas licensure as a Registered Nurse

Preferred:

  • Prefer at least one year experience in utilization review, resource management, discharge planning or case management.

  • Bachelor of Science in Nursing

Seeking a Case Manager Coordinator for the Emergency Department to join our team who will be responsible for facilitating effective & appropriate health services throughout the continuum of care. Collaborative and systematic processes are used to assess patient needs, plan services and collect data. Additionally, this position assists in the coordination, implementation, monitoring and evaluation of options and services to meet the individual's healthcare needs. Communication with physicians, payers and others to promote fiscal responsibility and effective use of available resources to decrease fragmentation of care across all settings. Acts as a resource for staff and other departments. * Keeps current on JC standards, CMS (Conditions of Participation, Core Measure, etc.). Review criteria and other rules that govern the provision of care, planning and documenting. * Tracks resource or other information used to identify improvement opportunities. Attends medical staff, hospital committee meetings and/or discusses findings with appropriate staff. * Conducts special projects to reduce patient's length of stay such as tracking avoidable days in relationship to high risk DRGs (Heart Failure, COPD and Sepsis). Education: * Graduate of an accredited Baccalaureate Degree Nursing Program Experience: * 5 years RN experience in acute care case management, including knowledge of: Diagnosis Related Group (DRGs) and ability to assess clinical data to determine a working DRG; appropriate intervention strategies to reduce length of stay and manage utilization; and reimbursement mechanisms and managed care issues that impact medical necessity and discharge planning.
