Registered Nurse (Rn) Case Manager Homecare

King David Post Acute Nursing & Rehabilitation - Full time Registered Nurse (RN) Case Manager for Homecare. Paid per visit.

Covers beachwood and surrounding areas. Will assist with admissions/revisits/discharges from homecare. Will have on call responsiability.

Competitive wages. Registered Nurse (RN) Case Manager DUTIES AND RESPONSIBILITIES: Patient Care Completes an initial assessment of patient and family to determine home care needs.

Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis.

Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures.

Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act. Counsels the patient and family in meeting nursing and related needs.

Provides health care instructions to the patient as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient from Homecare. Performs patient care such as placement and removal of indwelling urinary catheters, wound care, filling medication dispenser systems and other procedures covered under licensure.

Prepares equipment and materials for treatments, observing aseptic technique as required. Provides accurate and timely documentation of care using agency approved software to enter evaluations and visit documentation in accordance with agency policy and pay source requirements. Participates, as requested, in orientation of new staff, and continuing education.

Performs all other duties as assigned. Communication Prepares clinical notes and updates the primary physician when necessary and at least every 60 days. Communicates with the physician regarding the patients needs and reports any changes in the patients condition; obtains/receives physicians orders as required.

Communicates with community health related persons to coordinate the care plan. Registered Nurse (RN) Case Manager Additional Duties Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for homecare equipment and other necessary items and services are available.

Instructs, supervises and evaluates home health aide care provided every two (2) weeks. Registered Nurse (RN) Case Manager QUALIFICATIONS: Graduate of an accredited physical therapy assistant program with either an Associates degree in science or applied science that is approved by American Physical Therapy Association, Inc. Current certification or license as required by state law.

Demonstrates good verbal and written communication and organization skills. Possess and maintain current CPR certification. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements. IND123
