Regional Hospital is looking for a RN Case Manager who is a good team-player yet can function independently and be able to float to all departments having Case Management needs. Candidates must be flexible with schedule to work 8- hour day shifts, weekends and holidays as well as a once a month rotating call schedule.
Position summary: This position is designed to promote the achievement of optimal clinical and resource outcomes.
Responsible for facilitating appropriate lengths of stay and reimbursement for all hospital admissions in accordance with its goals and objectives. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. The Case Manager is accountable for a designated patient caseload and plans effectively in order to meet the patient needs, manage length of stay, and promote efficient utilization of resources.
Essential educational/licensure, skills and experience:
Registered Nurse with 2 years minimum med/ surgical nursing experience
Must have current certification as a BLS healthcare provider
Knowledge of community resources
Valued but not required educational/licensure, skills and experience:
BSN in the State of Indiana
Certification in Case Management
Prior experience in Utilization Management, Home Health Care, Nursing Home Management
Prior experience in insurance/certification review
Knowledge of Interqual Criteria or Milliman guidelines
Knowledge of Meditech and MIDAS systems
CPI Certification a plus
