Registered Nurse (Rn), Case Manager - Continuity Of Care

SUMMARY: The RN Case Manager, through patient assessment and engagement, chart review and collaboration with the healthcare team, develops an appropriate discharge plan, making recommendations for alternative levels of care, identifying cost effective care options and facilitates appropriate clinical follow up.

The RN monitors patient admissions and continued hospital stays for appropriateness and continued medical necessity, releasing information to third party payors as needed. Ability to stand and walk, bend, squat and twist, and occasionally lift up to 20 pounds with proper body mechanics.

Must be able to handle critical and highly stressful situations with efficiency and composure. Work Environment: Office setting, within a hospital/nursing unit environment
