What services does an RN Case Manager provide?

  • One-on-one nursing care with a flexible schedule
  • Assess clients physical and emotional status, level of competency, home environment, safety factors, caregivers ability to assist with care and needs of the client on admission and through all phases of care.
  • Formulate a nursing care plan in collaboration with client, caregiver(s) and physician including goals of care and treatment plans to attain these goals.
  • Reconcile medications on admission and every visit. Prepare, monitor and educate on all medications, reporting any discrepancies or side effects to physician immediately.
  • Educate and instruct on diagnosis and contributing factors to disease process including prevention and maintenance of illness or chronic disease.
  • Obtain and execute physician orders and keep physicians informed of all pertinent information concerning the clients condition and response to treatment.
  • Develop personal plan of care for clients and provide supervision that aligns with the personal care plan to LPNs, CNAs and PSS .
  • Effectively plan, organize, set priorities and complete paperwork and daily assignments in a timely manner with minimal direct supervision per agency policy.

Requirements

  • Current Registered Nurse License in the state of Maine
  • Holds a current CPR certification.
  • Minimum of one year licensed, 2 years of Nursing experience preferred.
  • Good organizational and planning skills.
  • Clear, respectful communication skills.
  • Good professional boundaries.
  • Able to work autonomously within the home environment.
  • Valid Driver's License
  • Reliable Transportation with Auto Insurance

For the clinical staff, this means there is the opportunity to share their clinical knowledge with and learn from new providers. The care provided ranges from birth to geriatrics including obstetrics and pediatrics. This position is perfect for a nurse who likes to stay busy and enjoys a variety of tasks including triage calls and direct patient care. The primary focus of this position will be to meet with OB patients for their initial obstetrical visit to review expectations during pregnancy as well as facilitate group visits with OB patients. The ideal candidate will have: * Active Maine RN licensure or ability to obtain same in a timely fashion * 2+ years of previous RN experience, preferred * Previous practice and/or OB experience, helpful * Computer skills including prior use of EMR * Excellent teamwork, communication and collaboration skills * Strong organizational skills and ability to multitask Job Description: Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes andmaximize available resources. Monitors, records, and communicates patient condition as appropriate. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Takes calls from patients/caregivers requesting assistance. Provides advice, referrals, and patient education. For symptomatic patients determines the necessity and type of care needed and advises caller regarding appropriate care. Performs administrative duties to coordinate patient care, including data collection, answering phones, scheduling tests/procedures and appointments, calling in prescriptions, determining insurance coverage, and billing and receiving payments.
