Part Time Day Shift
3 - 8 hour day/week between hours of 8am
4:30pm
No weekends/holidays/on-call
Will be required to travel to different locations. Primarily 3 locations in the Jackson area, 1 location per day. Could be asked to travel to other locations outside of Jackson for vacation/etc coverage.
Brief Description of day-to-day responsibilities: Perform 6-7 Annual Wellness visits per day.
Approx. 40 min per visit. Full assessment, Cognative Testing, medication Review, Look for Annual Labs they may need, some scheduling, vitals, etc.
Ideal Candidate: Minimum of 1 year RN experience in an ambulatory or clinic environment, maybe a home health nurse.
Experience with vitals, documentation in EPIC, immunizations. May work in-basket.
Henry Ford Health Jackson is a Magnet Designated Hospital. BSN is required or ADN with a commitment to enroll in a BSN program within 3 years, obtain your BSN within 5 years.
GENERAL SUMMARY: Provides leadership in the coordination and delivery of quality compassionate patient care.
Utilizes the nursing process of assessment, planning, implementation, and evaluation to provide, delegate, supervise and document care and teaching of patients and family. Effectively communicates with the health care team. Demonstrates clinical competence. May require a professional certification which is a regulatory requirement in the State of Michigan or a documented community standard for the position, works independently and in collaboration with principal investigator when appropriate.
EDUCATION/EXPERIENCE REQUIRED: Graduate from an accredited school of nursing.
Bachelor's Degree preferred. Successful completion of Henry Ford Ambulatory orientation. Maintains and demonstrates current clinical knowledge in area of specialty.
Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans. Ability to perform a wide variety of professional nursing services. Ability to effectively communicate with patients, families, and medical group personnel.
Preferred experience in the specialty. Basic computer knowledge and keyboarding skills.
CERTIFICATIONS/LICENSURES REQUIRED: Current licensure by the Michigan State Board of Nursing as a Registered Nurse.
Current BLS or ACLS per Henry Ford Health policy. Certification will be required when it is a regulatory requirement in the State of Michigan or a documented community standard, i.e., Oncology Certified Nurse (OCN).
