Distinguishing Features of Work
An employee in this class is responsible for reviewing client needs as required according to federal and state program eligibility criteria. Duties may include providing comprehensive assessments, educational counseling, and providing case management and care coordination depending on the program area. The work is performed under the direction of assigned supervisory staff.

Selection Process
MINIMUM QUALIFICATIONS (Pass/Fail):
Graduation from an accredited college or university with an Associate's degree in nursing.
Possession of a current Minnesota Registered Nurse license.
Possession of a valid driver's license.
ADDITIONAL REQUIREMENTS BY PROGRAM AREA:
Home and Community-Based & Public Health Managed Care Programs:
Two (2) years full-time paid verifiable experience in a home or community-based setting.
Possession of MnCHOICES Certified Assessors certification within two (2) months of employment. Failure to obtain the MnCHOICES Certified Assessor certification within two (2) months will result in termination from the position.
RATING SYSTEM:
Experience and Training rating will be 100% of the scoring process:
Points will be awarded for job-related experience and education exceeding the minimum qualifications. Please be thorough in listing your work experience in the Work Experience section of the application as this will be used to determine part of your score. Only experience occurring during the ten most recent years will be used in determining this assessment. Experience beyond ten years may be included on the application for purposes of meeting minimum qualifications or informing the hiring authority of your complete employment history.
