Tasks assigned to this position involve exposure to blood and body fluids.


  • Acts as a charge nurse in the assigned Health Care Center.

  • Incumbent will assist the Supervisor, as directed, in planning, organizing, developing, and directing day-to-day operations of the Center.

  • Participate in the assessment of resident needs and the development and coordination of resident care plans.

  • Perform those nursing procedures requiring Registered Nurse designation, as well as some managerial tasks, with a customer service focus.

  • Performs procedures to include the following in accordance with Blue Skies policies and generally accepted nursing practices: Medication administration and recording; administer oxygen; insert and irrigate Foley catheters; provide tracheotomy care; perform Heimlich maneuver; provide decubitus assessment and care; perform gastrostomy tube procedures; perform cardiopulmonary resuscitation (CPR); utilize Automated External Defibrillator (AED).


  • Must be able to supervise and work harmoniously with professional staff and interact effectively with resident's family, physicians and other professionals, internally and externally.

  • Incumbent must be able to speak, read, write, and follow verbal and written instructions in English.

  • Is expected to perform duties with only general supervision and exercise independent action to meet the requirements of the position.

  • The ability to communicate in Spanish is helpful, but not essential.


  • Required: Must be a Registered Nurse licensed by, and in good standing with, the Texas Board of Nurse Examiners.

  • Preferred: Some education beyond the minimum licensing level is desirable. At least one year of previous experience in a long term care setting.


  • Must be able to satisfactorily demonstrate knowledge of, and train others in the use of, professional nursing equipment and supplies and simple and complex medical equipment to include patient lifts, hospital beds, nurse call systems, and whirlpool baths.

  • Basic computer knowledge is essential, as well as the ability to operate general office equipment such as copier, telephone, and fax.

  • Training will be provided to familiarize incumbent with work-related equipment to ensure safety and proper operation during use.


  • Required: Must have current CPR and AED certifications. Must be current in attaining the Continuing Education Units necessary to meet licensing requirements and participate in continuing education programs, when directed.

  • Preferred: Specialized training in nursing, supervisory skills, and infection control, is desirable.


  • Supervises and provides nursing care according to physician orders and in conformance with recognized nursing techniques and procedures, established standards, and administrative policies.

  • Assigns tasks to unlicensed personnel.

  • Records all pertinent treatment information on assigned residents to include observations, treatments, and medications in Electronic Medical Records (EMRs); keeps Supervisor informed of any adverse changes in condition of same.

  • Makes periodic rounds of each assigned resident, notes needs and conditions, and reports adverse situations, as applicable, to the Supervisor and Physician.

  • Receives Physician orders and enters them in EMRs, as needed.

  • Maintains required medical records and administers medications and treatments according to State laws and regulations and standard nursing practices.

  • Provides oncoming charge nurse with resident reports and receives same from departing shift, as required.

  • Performs routine nursing duties to include: baths, oral hygiene, temperature, pulse, respiration, blood pressure, nutrition and diet management and assessments, feeding, exercise, dressing changes, specimen collection, and postmortem care.

  • Performs other general tasks as required to provide for the safety and physical, spiritual and emotional comfort of residents.

  • Knows and follows established emergency procedures; administers appropriate medical care, notifies appropriate officials, carries out Physician orders, and provides leadership and proper response.

  • Attends continuing education courses, seminars, and in-service programs, as required.

  • Provides accurate inventory of medications and dispenses same without error.

  • Performs other duties as assigned by Supervisor.



  • Within the scope of position duties, responsible for seeing that operations are effective and efficient, assets are safeguarded, and applicable laws, regulations, policies and procedures are complied with.

  • This position will handle confidential information and must treat such with appropriate sensitivity.


  • Work is performed indoors in a climate-controlled facility.

  • Bending, squatting, and reaching are necessary, as well as occasional lifting and transfer of light loads of less than 30 pounds.

  • Incumbent must be able to make walking rounds of the Health Care Center and be able to lift 150 pounds or more, with the assistance of another employee, in order to respond to emergency medical situations and be able to push an occupied bed and occupied wheelchair a few hundred yards unassisted.

  • Must have eyesight correctable to 20/20 and hearing within the normal range to ensure the incumbent can quickly and accurately respond and make patient assessments to medical emergencies in work areas.

  • Because of the health care setting, the use of chemicals and cleaning products is carefully controlled.

  • Few environmental hazards present, although the threat of communicable disease exists.

  • Mild cleaning products, waxes, and soaps are used in the immediate area.

  • Mild pesticides are used periodically to control bug infestation.


  • Work is normally scheduled to coincide with one of three standard nursing shifts to allow coverage for a 24-hour, 7 day-a-week operation.

  • Exact duty hours are established by the Director of Nursing and the Administrator, and the employee is advised of the assigned schedule in advance.

WORK LOCATION: Assigned Health Care Center.


  • Prescribed nursing uniforms or scrubs are appropriate for wear.

  • Slip-resistant, closed-toe shoes required.

  • Blue Skies employee ID badge or name tag must be worn at all times while on duty.

  • Gowns, gloves, safety belts, and other items of personal protective equipment clothing are worn, as needed, and training will be provided to ensure safety and proper wear during use.
