Summary

Plans, coordinates and provides total nursing care for residents and provides supervision and guidance to clinical staff members. Scope of work may be modified by state specific rules under the Nurse Practice Act.

Qualifications

  • Education: Graduate of accredited RN School of Nursing.

  • Licenses/Certification

  • Valid RN license in the state employed.

  • Valid CPR certification.

  • Experience: One year of nursing experience preferred.

Essential Functions

  • Initiates and leads team conferences in development of individualized nursing care plans.

  • Assesses and documents the resident's condition and nursing needs.

  • Accurately and promptly implements physicians' orders.

  • Assigns nursing care to team members in accordance with the resident's needs and the person's capabilities and qualifications.

  • Supervises, directs and evaluates LPNs, LVNs, and CNAs.

  • Administers medications, starts IVs and performs treatments for assigned residents, and documents that treatment as required company, and local, state and federal rules and regulations.

  • Participates in orientation and in-service training for personnel.

  • Assists physicians with special tests and procedures within the scope of state specified Nursing Practice Act.

  • Ensures that supplies are utilized economically and that equipment is clean and maintained in a safe manner.

  • Reports and records pertinent observations and reactions regarding residents.

  • Coordinates nursing care of residents when scheduled for therapy or procedures by other departments.

  • Assists with or institutes emergency measures for sudden adverse developments in residents.

  • Interviews applicants and recommends hires. Participates in counseling, disciplinary action and termination of staff, to extent permitted by state practice act.

  • Supervises nursing unit, which includes scheduling and directing the nursing staff to extent permitted by state practice act.

  • Ability to work cooperatively as a member of a team.

  • Ability to communicate effectively with residents and their family members, and at all levels of the organization.

  • Ability to react decisively and quickly in emergency situations.

RN with 2 years nursing experience in acute care setting or rehab setting is needed in this role.# Hospice or Home Care experience is highly preferred.## ## Essential Functions: Follows up and interacts with patients/families/caregivers and employees in a courteous and professional manner at all times to ensure that an excellent experience is received by all that will contribute to Hospice of Michigan#s status as a world class hospice. Completes initial history and assessment, documenting problem areas to be developed in the plan of care within twenty-four (24) hours of enrollment. Assists in developing, implementing, evaluating and updating the individualized plan of care and recommends revisions to the plan as necessary based on acuity and patient/family/caregiver needs, including but not limited to, referrals to the appropriate discipline. Completes initial and on-going assessment of the impact of the terminal diagnosis on the patient#s physical, functional, psychosocial and environmental needs and activities of daily living, including:# the risk for pathological grief, cultural and spiritual implications, and verbal and non-verbal communication patterns. # Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and to assist the physician in determining the patient#s eligibility for hospice and determines scope and frequency of services needed based on acuity and patient/family needs. Provides end-of-life care including comfort care and symptom management according to Hospice of Michigan indicators. Documents patient care following established Hospice of Michigan#s standards of documentation practice, regulatory and licensure requirements, and submits documentation in accordance with Hospice of Michigan#s policy. Provides initial and ongoing patient/family instruction, education and support regarding the disease process, self-care techniques, end-of-life care, and issues of ethical concerns. Assesses the ability of the caregiver to meet the patient#s immediate needs upon admission and throughout care. # Qualifications: Graduation from an accredited school of nursing with possession of a current license to practice as a registered professional nurse in the State of Michigan. Minimum of one (1) to two (2) years of varied nursing experience required; hospice experience preferred. Skilled in nursing practice with a demonstrated ability to assess and respond to the needs of patients and families.
