RESPONSIBILITIES:

  • Assess, as needed, patients

individual needs on the basis of assessment and data collection to ensure

proper admission per funding regulations.

  • Perform client medication data entry

necessary to complete admission using the Electronic Health Record as

required by Program Manager/Director.

  • Assist Case Managers to develop

treatment plans that are coherent and reflective of clients progress and

development. Involve client

appropriately in the process and document in the Case Note section of the

Electronic Health Record.

  • Implement doctors orders, administer

specified medication, monitor and record vital signs as needed, and

maintain medical/ nursing records.

  • Observe patients and document their

responses to treatment and report out these findings to staff in daily

staff via email updates.

  • Carry out administrative procedures in

compliance with program policies.

  • Provide medical documentation and

medication monitoring training to staff.

  • Participate in Case Management

treatment team weekly meeting providing patients progress and treatment.

  • Monitor clients in individual, group,

and family formats.

  • Discharge patients in accordance with

established policy.

  • Respond to client crises and

emergencies as needed.

REQUIREMENTS:

EDUCATION

  • Knowledge

obtained through completion of a Bachelor Degree curriculum from an accredited

institution, orequivalent combination of knowledge and experience in substance abuse.

EXPERIENCE

  • Minimum one year related experience

KNOWLEDGE/SKILLS/ABILITIES

  • Ability to perform the core functions of a

substance abuse health professional.

  • Ability to develop rapport with

substance-abusing populations and families.

  • Ability to apply organizational management

skills and maintain self and client discipline.

  • Ability to prepare and maintain required

documentation.

  • Ability to comply with internal, local,

state, and federal regulations and policies.

  • Ability to work and communicate with

others.

  • Ability to work within the confines of a

modified therapeutic community.

VIEW JOBS 3/11/2019 12:00:00 AM 2019-06-09T00:00 Healogics is hiring for a PRN Registered Nurse/ RN to join our Wound Care Center team in Holyoke, MA Under the direction of the Wound Care Center Clinical Nurse Manager/Clinical Coordinator, the Registered Nurse (RN) primarily provides patient care and handles intake and discharge processes for the Wound Care Center patients as scheduled. The position may also serve as a Case Manager to a group of assigned Wound Care Center patients, as assigned and depending on the Wound Care Center setup. All Healogics employees should perform their job responsibilities according to Healogics clinical policies, employee policies, and Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable. RN/Registered Nurse Responsibilities: Provides patient care to Wound Care Center patients and handles patient intake and discharge processes as assigned and according to the hospital and nursing standards. May also serve as Case Manager to a group of patients as assigned. This entails: Performing the initial nursing assessment, communicating findings with the assigned physician, assessing the wound stage and measurements, documenting care plans, evaluating the patient’ status in regards to desired outcomes, and identifying patient care situations that require intervention Participating in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of the patient care Performs hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of the Healogics hyperbaric training. Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and the Medical Director regarding clinic and patient needs. Performs other duties as required.
