You can make an exceptional difference in the lives of our patients and their families dealing with end-stage renal failure or chronic kidney disease.
If you haven't considered Nephrology before, read on as we think that you should. Dialysis Experience is NOT required.

  • Long term patient relationships. Build meaningful relationship with patients and their family in an intimate outpatient setting.

  • A team that feels and functions like a family. Staff RNs are a central part of a small interdisciplinary team of clinicians. Work alongside and support technicians, other nurses, dietitians, social workers, physicians and the center manager.

  • Specialized, complex care. RNs deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.

  • Fast paced. Our RNs oversee a group of patients (census dependent on state laws) with the support of PCTs to conduct patient observations, measuring stats and machine set up. You will work with your head, heart and hands each day.

  • Schedule. Shifts are typically 10-12 hours, 3-4 days per week. Schedule times will vary by locations but most of our RNs work early morning or days shifts. Some holidays required.

  • Some details about this position:*

  • Training may take place in a facility or a training clinic with a class of new teammates other than your assigned home clinic

  • You must be flexible on the final home clinic placement it may be in a different location; there is also the potential to float to various clinics during and after your training

  • You must have a flexible schedule and be able to work mornings, evenings, weekends, and holiday

  • Current Registered Nurse (RN) license in the state of practice; Graduate Nurse pre-licensure available in some locations

  • A desire to deliver care in an empathetic, compassionate way. You know, the way you'd want your own care to be

  • Experience assessing, trouble shooting and making sound recommendations in stressful situations

  • Excellent communication skills to listen and communicate with patients and teammates

  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree

  • Basic computer skills and proficiency in MS Word and Outlook. You will be required to chart patient's assessment, orders and your care notes in our electronic medical record system.

  • You might also have (a.k.a. nice-to-haves):

  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience

  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)

Uses the nursing process and evidenced based practice to collaborate with the core PACT Team (Primary Care Provider, LPN/LVN and Medical Assistant) and expanded PACT Team including family/ caregiver, VA, and community based services involved in developing the patient care plan. Core Responsibilities * Patient Care: Actively assists practitioners with patient care, physical examinations and ancillary tests. * Practices specialized skills in patient assessment such as but not limited to: triages, blood pressure checks, injections, insulin teaching via telephonic and direct contact with patient in order to facilitate care transition internally as well as externally meet the patients' need * Triages unscheduled patients requesting care the same day by identifying acute patient needs and mobilizing a care team for optimal patient outcomes. * Acts as point person within PACT Team, by coordinating daily huddles, identifying high acuity patients and identifying Veterans at risk for uncoordinated care * Assists in co-managed care collaboration with non-VA providers, and facilities. * Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's total health goals. This may include patient teaching, preparing education materials and providing individual management of patients needing extra support, including those with chronic conditions. * Monitors VA discharge list and contacts patients for follow up after hospital or other facility discharges. * Maintains confidentiality of veterans' data and information. * Complies with all VA and company training requirements. * Consults as permitted by VAMC. * Compliance: Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items. * Clinical Outcomes: Remain focused on achieving excellent clinical outcomes through the specified VA guidelines. * Assists administrator in maintaining professional and educational competencies of staff. * Assists Clinic Operations Director as back up "Nurse in Charge" in times of COD absence. * Leads regular Nurse staff meetings and acts as mentor and resource to nursing staff. * Customer Service: Must provide excellent customer service to each veteran, both in person and over the phone, as well as to fellow colleagues and clinic visitors. * Establish and maintain a warm, welcoming and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met. * Collaborates with Clinic leadership to resolve veteran concerns or complaints. * Enrollment Growth: Participate in the clinic's outreach events to help support the clinic's enrollment initiatives. * Save & Recapture: Support patient enrollment and retention in appointments, follow-up visits or nurse visits. * Strategic Initiatives: Embrace and support new initiatives, whether clinical or operational. Requirements * Bachelor of Science in Nursing (BSN) required; if VA contract allows, may be Associate Degree RN. * Minimum 2-3 years' experience as a registered nurse in a related primary care or ambulatory care setting; government healthcare environment preferred. Specific requirements could vary based on individual VA contract. * Must be credentialed and remain in good standing through the Veterans Health Administration (VA). * Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements. * Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency - AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract * Proficiency in written and spoken English. * Strong computer skills, including EMR experience * Energetic and optimistic demeanor * Strong service mentality and a focus on achieving all aspects of defined service standards * This is considered a safety sensitive position.
