Optimal Home Care is looking for a Registered Nurse to provide services to our patients in the Loveland and surrounding areas.

About the Position:

The goal of the home health Registered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes.

When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems.

Pay: $75,000-100,000

• Optimal Pays Per Visit*

Qualifications

Qualifications:

• A graduate of an accredited, approved school of professional nursing

• Active license in the State of Colorado in good standing.

• Must be able to communicate in the language of the client and document client related-activity as appropriate

• Current driver's license

• At least one (1) year of experience as a Registered Nurse recently in Home Health is required.

• Familiarity with OASIS required.

• Due to the requirements mandated by the Colorado Board of Health, you must be fully vaccinated to be considered for this position with Optimal Home Care, Inc.
